Responsibilities
- Perform comprehensive physical therapy evaluations in the home setting
- Develop individualized treatment plans based on patient goals and clinical needs
- Provide skilled physical therapy services in patients' homes
- Help patients improve strength, mobility, balance, endurance, and functional independence
- Assess home environments and incorporate them into treatment plans to maximize outcomes
- Educate patients and caregivers on exercises, safety techniques, equipment use, fall prevention, and injury prevention
- Collaborate with physicians, nurses, occupational therapists, speech-language -pathologists, and other healthcare professionals
- Monitor patient progress and modify treatment plans as needed
- Maintain accurate and timely clinical documentation
- Complete patient notes and required communications within agency timeframes
- Ensure compliance with agency policies and applicable regulations
- Travel to patient homes within assigned coverage areas

Qualifications
- Active Pennsylvania Physical Therapist (PT) License
- Graduate of an accredited Physical Therapy program
- Home Health experience preferred but not required
- Strong clinical assessment and treatment skills
- Excellent communication and interpersonal skills
- Strong computer and documentation skills
- Reliable transportation and valid driver's license
- Ability to work independently and effectively manage a flexible schedule
- Knowledge of electronic medical record systems is a plus
- Commitment to delivering exceptional patient-centered care
