What You’ll Do
Support Residents & Families: Provide compassionate counseling, guidance, and emotional support to residents and families while helping them navigate transitions, care planning, and healthcare recommendations.
Facilitate Admissions & Transitions: Assist with the admissions process, welcome new residents and families, and help ensure smooth, supportive transitions into the community.
Collaborate with Interdisciplinary Teams: Partner with physicians, nursing staff, and healthcare professionals to address the social and emotional factors impacting resident wellbeing and participate in care planning meetings.
Manage Social Services Documentation: Complete social histories, assessments, discharge planning, and required documentation accurately and in compliance with organizational and regulatory standards.
Advocate for Resident Needs: Assist families with Medical Assistance applications and connect residents with appropriate resources, services, and support systems.
Promote Education & Communication: Provide training and education for staff while maintaining positive relationships with residents, families, colleagues, and community partners.
Ensure Professional Excellence: Maintain resident confidentiality, uphold professional standards, and contribute to a compassionate, collaborative, and resident-focused environment.

Requirements:Qualifications
Required:
- Bachelor’s degree in Social Work (BSW) from an accredited college or university.
- One to two years of related experience, or an equivalent combination of education and experience.
- Strong interpersonal, communication, and relationship-building skills.
- Ability to effectively support residents, families, and interdisciplinary teams in sensitive and emotional situations.
- Computer proficiency and ability to complete documentation accurately and efficiently.
- Ability to interpret regulations, write reports, and communicate effectively with a variety of audiences.
Preferred:
- Experience working with older adults in long-term care, healthcare, or senior living environments.
- Knowledge of geriatric mental health services and resident-centered care practices.
