JOB REQUIREMENTS

Education
-
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited institution.
- Current unrestricted Registered Nurse (RN) licensure in the State of Arkansas.
Licensure/Certifications:
Preferred:
- Certified Case Manager (CCM), Orthopedic Nursing Certification (ONC), or related specialty certification preferred.

Experience

Required:
- Minimum of two (2) years of clinical nursing experience in acute care, outpatient care management, orthopedic services, case management, or population health.
- Experience in care coordination, utilization review, chronic disease management, or quality initiatives preferred.
Preferred:
- Experience working within Joint Commission accredited healthcare organizations preferred.
- Knowledge of CMS quality metrics, population health strategies, and evidence-based fracture prevention practices preferred.

- Care coordination and patient navigation

JOB SUMMARY

The Case Manager – Bone Health & Fracture Prevention Clinic is responsible for coordinating comprehensive, evidence-based patient care services for patients identified as at risk for osteoporosis, fragility fractures, and bone health complications. This role supports continuity of care, regulatory compliance, patient education, and interdisciplinary collaboration to improve clinical outcomes, reduce readmissions, and promote quality-driven, cost-effective healthcare delivery.
The Case Manager serves as a clinical liaison between providers, patients, caregivers, diagnostic services, and community resources to ensure patients receive timely evaluations, treatment coordination, follow-up care, and preventative interventions aligned with Joint Commission standards, CMS quality expectations, patient safety initiatives, and evidence-based orthopedic and population health practices.
