Job Description
GENERAL SUMMARY/BASIC PURPOSE OF JOB: Under supervision, the Community Health Worker I (CHW I) provides health education, disease management education, health care and social service navigation services, and/or case management services to patients/community residents
MINIMUM EDUCATION:
High school diploma or GED. MINIMUM WORK EXPERIENCE:
One year of health education, health care, or related experience preferred.
KNOWLEDGE & SKILLS:
Ability to provide health education, one-on-one and in groups.
Ability to provide case management under supervision.
Familiarity with and/or experience with working in Chicago’s West Side and/or South Side communities (resident of West Side or South Side communities preferred).
Passion for and dedication to working with community members and providers.
Ability to connect and build trust with diverse patient/participant population.
Demonstrated commitment to racial and health equity.
Punctual, reliable, and willing to learn.
Working understanding of Microsoft Word, Access, Excel, and PowerPoint preferred.
Valid driver’s license and own car preferred (required for some positions).
Bilingual in Spanish may be required for some positions.
* Hours are flexible but will include some evening and weekend hours scheduled in advance. .
