Position Summary:

Reporting to the Primary Care Nurse Manager, the Family Planning Coordinator is responsible for leading, coordinating, and delivering comprehensive family planning services, including patient counseling, clinical support, care coordination, and program operations and workflow optimization related to contraception, pregnancy options, and sexual health education. This role serves as a key clinical and operational resource, ensuring high-quality, patient-centered care delivery and efficient program performance across the Family Planning program.

Primary Responsibilities:

  • Family Planning Counseling & Clinical Services
    • General:
      • Inform patients of the wide array of contraceptive methods available
      • Provide information and supportive counseling to assist patients in making their own decision regarding unplanned pregnancies
      • Follow up on family planning patients via phone calls, correspondence, MyChart messages, and other outreach methods
      • Provide pregnancy tests, STI screening, and HIV pre- and post-test counseling as needed
      • Consult with providers regarding patient status as necessary and refer clients to other services within the clinic including prenatal registrations as needed
      • Serve as a subject matter resource for family planning services, supporting clinical staff with guidance on workflows, counseling approaches, and available resources
    • Initiate and conduct Family Planning visits including:
      • Pregnancy testing and education
      • Pregnancy options counseling, resources, support, and follow up
      • STI testing and education
      • Contraceptive counseling and education
      • Contraceptive provision (Depo, pill/patch/ring/emergency contraception) and scheduling for LARCs (IUD, Nexplanon)
      • Ensure consistent, high-quality delivery of family planning services through adherence to evidence-based protocols and best practices
    • Support MA/Provider-initiated care through warm handoffs and asynchronous follow-up:
      • Respond to warm handoffs initiated by MAs for patients screening positive for contraceptive counseling needs
      • Respond to provider-initiated warm handoffs during visits for counseling, education, and consent completion
      • Conduct outreach outside of visits as requested by providers (e.g., follow-up, refill requests, lab results, counseling, scheduling, and care coordination)
    • Provide LARC Clinic coordination:
      • Schedule patients for LARC procedure (IUD, Nexplanon)
      • Review and optimize LARC Clinic schedule to ensure appropriate booking and adjust/rebook as needed
      • Conduct reminder calls and pre-appointment counseling calls
      • Provide counseling, obtain consent, and collect relevant history on day of appointment
      • Ensure adequate supply of instruments and LARC devices in coordination with clinical team
      • Partner with clinical leadership to optimize LARC Clinic workflows, access, and utilization
  • Outreach and Collaboration
    • Actively participate in outreach and collaboration activities with other agencies and CSHC departments
    • Instruct individuals and groups in family planning issues
    • Provide services to schools as health educator around issues of self-esteem, values, sexuality, hygiene, HIV related issues and choice
    • Work with ABCD and other funding sources; comply with all ABCD policies, procedures, protocols, and administrative requirements
    • Collaborate with the clinical team to develop and distribute patient-facing and program materials about family planning services provided by CSHC
    • Participate in community health projects such as health fairs, school health initiatives and others
    • Represent the Family Planning program in internal and external meetings, partnerships, and initiatives as appropriate
  • Care Coordination & Administrative Support
    • Close visits with proper procedure and documentation
    • Report all required data to funding sources
    • Complete monthly beneficiary reporting form
    • Forward all non-billable encounters to the finance department
    • Attend point-of-care testing competency training
    • Perform point-of-care urine HCG testing (as applicable)
    • Stock condoms weekly in all departments
    • Monitor and manage inventory of family planning supplies
    • Coordinate ordering family planning supplies for primary care
    • Coordinate appointment scheduling for family planning and LARC visits
    • Assist family planning patients with check-in, scheduling, and other administrative tasks
    • Identify opportunities for process improvement and collaborate with leadership to implement solutions that enhance efficiency, access, and patient experience

Requirements:

Qualifications

  • Three or more years of experience in a Family Planning program or similar clinical setting preferred, with demonstrated experience in patient counseling, care coordination, and program support in reproductive health, sexuality, contraception, and pregnancy options.
  • Demonstrated ability to lead workflows, manage program operations, and support quality improvement initiatives strongly preferred.
  • Experience supporting clinical workflows and interdisciplinary coordination strongly preferred. Candidate is required to successfully complete a basic family planning counseling course and certification by ABCD/BFP.
  • Skills and Competencies
  • In-depth knowledge of healthcare quality improvement principles, including performance measurement and population health management.
  • Expertise in Epic EHR workflows, with the ability to train others on system use and documentation standards.
  • Optimal use of Microsoft Office Suite
  • Excellent interpersonal skills, with a focus on patient engagement and team collaboration.
  • Strong problem-solving abilities, particularly in workflow optimization and process improvement.

Physical Requirements:

· Must be able to stand or sit for prolonged periods (at least 50% of the time)

· Ability to lift up to 25 pounds and load onto shelves

· Visual acuity sufficient for frequent reading and computer use
