Concept Rehab is seeking a dedicated Speech-Language Pathologist (SLP) to join our rehabilitation team in a skilled nursing and post-acute care setting. This role offers the opportunity to work with adult and geriatric patients experiencing communication, cognitive, and swallowing challenges following illness, injury, neurological conditions, or hospitalization.

As an SLP, you'll play a key role in helping patients safely communicate, participate in daily activities, and maintain the highest possible quality of life.

Requirements:

What You'll Do

  • Perform comprehensive speech, language, cognitive, and swallowing evaluations
  • Develop individualized treatment plans based on patient needs and functional goals
  • Deliver skilled interventions for communication, cognitive-linguistic, voice, and dysphagia disorders
  • Assess swallowing safety and recommend appropriate dietary modifications when indicated
  • Collaborate with nursing, physicians, dietitians, rehabilitation professionals, and caregivers
  • Educate patients and families regarding treatment strategies and therapeutic recommendations
  • Monitor patient progress and modify treatment approaches to maximize outcomes
  • Participate in interdisciplinary care planning, quality initiatives, and discharge planning
  • Maintain accurate, timely, and compliant documentation
  • Support evidence-based clinical practice and patient-centered care

Qualifications

  • Master's Degree in Speech-Language Pathology from an accredited program
  • Active Ohio Speech-Language Pathologist license
  • Certificate of Clinical Competence (CCC-SLP) preferred
  • Clinical Fellowship candidates encouraged to apply
  • Experience in skilled nursing, post-acute care, or geriatric rehabilitation preferred
  • Strong communication, problem-solving, and clinical decision-making skills
