Locations: Langhorne & Levittown, PA
Employment Type: Full-Time
Salary Range: $45,000–$50,000 annually
Family Service Association of Bucks County is seeking compassionate and dedicated Case Managers to join our team and help make a meaningful difference in the lives of individuals and families in our community. Each year, we support nearly 30,000 individuals as they navigate challenges including substance use, mental health concerns, homelessness, and other life barriers.

We are currently hiring for multiple full-time Case Manager roles, including:
- Blended Case Manager (BCM) – Langhorne, PA | $45,000 annually
- Shelter Case Manager – Levittown, PA | $45,000 annually
- Community-Based Case Manager (CBCM) – Levittown, PA | $45,000 annually
- Housing First Blended Case Manager (HFBCM) – Langhorne, PA | $50,000 annually

Key Responsibilities
Case Managers provide person-centered support, advocacy, and coordination of services to individuals experiencing homelessness and other complex challenges. Responsibilities include:
- Assist individuals in accessing safe, stable housing and community-based supports.
- Conduct comprehensive assessments and develop individualized service plans.
- Monitor progress toward housing stability, recovery, and self-sufficiency goals.
- Provide case management, advocacy, supportive counseling, and service coordination.
- Connect clients with community resources, benefits, childcare/preschool programs, income supports, and other essential services.
- Provide crisis intervention to promote the safety and well-being of clients, staff, and visitors.
- Support clients with budgeting, financial literacy, and household management skills.
- Participate in resident meetings, discharge planning, and housing transition activities as necessary.
- Maintain accurate and timely documentation according to funder and agency standards.
- Collaborate with internal teams, community partners, and service providers to ensure coordinated care.
- Utilize trauma-informed, Housing First, and harm reduction approaches to care.
Position-Specific Responsibilities
Blended Case Manager (BCM):- Complete assessments and develop individualized service plans.
- Provide ongoing outreach, service coordination, and advocacy in community-based settings.
- Link individuals to behavioral health, medical, educational, vocational, peer, and self-help resources.
- Coordinate services among multiple providers to support recovery and independence.
- Provide follow-up, aftercare coordination, and ongoing support based on client needs.
Community-Based Case Manager (CBCM):
- Provide outreach and service coordination in a variety of settings, including community locations, apartments, shelters, and offices.
- Support individuals with mental health and/or substance use diagnoses who have experienced chronic homelessness in obtaining permanent housing.
- Assess social determinants of health, housing history, and health-related needs.
- Coordinate with nursing and clinical teams regarding referrals and follow-up services.
- Advocate for clients and assist them in navigating multiple service systems.
Housing First Blended Case Manager (HFBCM):
- Support individuals experiencing homelessness in obtaining and maintaining housing.
- Provide ongoing case management focused on housing stability, recovery, and independence.
- Work collaboratively with community providers to ensure comprehensive support.
Shelter Case Manager:
- Provide case management services to shelter residents working toward permanent housing solutions.
- Assist residents with housing applications, community referrals, benefits, and discharge planning.
- Support a safe and supportive shelter environment through engagement and crisis response.
Qualifications
- Bachelor’s degree in Social Work, Human Services, or a related field, or an equivalent combination of education and experience.
- Valid driver’s license required.
- Experience in case management, shelter services, homelessness services, or related human services preferred.
- Computer proficiency, including experience with HMIS, Credible, ADP, and Microsoft Office Suite.
- Strong communication, crisis intervention, problem-solving, organization, and culturally responsive care skills.
