Job Description
Job Title: Patient Care Coordinator
Position Summary
The Patient Care Coordinator in a pain management call center acts as the primary liaison between patients, providers, and insurance companies. This role ensures patients receive coordinated, compassionate, and efficient care throughout their treatment journey. Responsibilities include scheduling appointments, managing patient communications, coordinating procedures, and supporting clinical and administrative operations.
Key Responsibilities
- Schedule and coordinate patient appointments, follow-ups, injections, and procedures.
- Maintain accurate patient records within the Electronic Medical Record (EMR) system.
- Communicate with patients regarding treatment plans, medical records, referrals, and appointment reminders.
- Coordinate care with physicians, nurse practitioners, pharmacies, imaging centers, and referring providers.
- Monitor provider schedules and ensure efficient patient flow.
- Handle incoming calls, patient concerns, and messages in a professional and empathetic manner.
- Ensure compliance with HIPAA and clinic policies regarding patient confidentiality.
- Support billing and collections by confirming demographic and insurance information.
- Educate patients on clinic processes, pain management services, and treatment expectations.
Preferred Skills
- Experience with pain management procedures and terminology.
- Bilingual abilities are a plus.
- E-Clinical Works EMR experience preferred.
Work Environment
- Fast-paced pain management call center.
- Frequent interaction with patients experiencing chronic pain.
