Job Description
As an Welcome Family Registered Nurse (RN)you will be responsible for providing home and community based services to infants (birth to 8 weeks).
The Welcome Family program offers a universal one-time visit (in the home or via telehealth) and follow-up phone call to all caregivers with newborns, regardless of age, income, number of children, or other criteria. The goal is to serve as an entry point into the early childhood system of care for families with newborns. The visit is conducted by a maternal child health nurse up to eight weeks postpartum, and ideally within two to four weeks after birth. Any caregiver is eligible, including fathers, grandparents, adoptive, and foster parents. The visit lasts approximately 90 minutes and takes place at the family’s home or a mutually agreed upon location. During the visit, the nurse identifies and addresses family needs through screening, brief intervention, education and support, and referrals to community resources.
Welcome Family is a free, voluntary program for mothers and their newborn babies. This is a universal program available to all mothers and newborns who reside in the Greater Lowell area or gave birth at Lowell General Hospital. Welcome Family provides mothers with a one-time nurse home visit, valuable community information and a Welcome Family gift bag.

Thom Child & Family Services offers competitive salaries and a comprehensive benefit package including, but not limited to:

Position Responsibilities:
- Assist and/or complete initial intakes and provide follow-up referrals and/or interagency service coordination as needed.
- The RN will focus on medical history, nursing assessment, as well as goals and objectives related to health, nutrition, and safety.
- Provide services using a coaching model with infants and families. This will include home visits, and may include child groups and parent groups.
- Monitor developmental milestones and progress with parents/caregivers to inform ongoing need for direct service, assessment and/or referrals.
- Provide parent/caregiver education on health and safety issues during home visits or parent education groups.
- Complete progress notes, reports, assessments, and correspondence as appropriate.
- Document all information necessary for the upkeep of client files.
- Participate in developmental screenings in the community.
- Attend program functions for clients and the public.
Position Requirements:
- Current Registered Nurse licensure in the state of Massachusetts
- BSN preferred
- Ability to work independently and as a team member
- Strong ability to engage in meaningful, therapeutic relationships with children, parents/caregivers, and community partners
- Knowledge of family systems and early childhood development
- Commitment to delivering individualized and culturally informed services
- Excellent communication and organizational skills
- Abide by all confidentiality and professional boundaries standards
- Good computer skills as well as strong documentation skills
- Current driver’s license and reliable transportation
- Bilingual/Bicultural a plus.
- CPR/First Aid required within the first 6 months
