Job Summary:
As an RN - LPN Clinical Care Coordinator, you will provide patient care and support through telephonic triage calls, electronic communication, and virtual health management. You will play a critical role in assessing patient needs, providing education, and coordinating care. You will support the healthcare team in managing patient care plans and ensuring seamless communication between patients, providers, and external agencies.
Key Responsibilities:
- Serve as the primary point of contact for patients requiring clinical support.
- Conduct remote patient assessments and provide education on care plans.
- Collaborate with providers to manage and coordinate patient care plans, ensuring timely follow-ups and interventions.
- Monitor and report patient labs.
- Complete prior authorizations and refill patient medications.
- Document patient interactions and updates in the electronic health record (EHR) system.
- Monitor and manage patient referrals to specialists, ensuring timely scheduling and follow-through.
- Maintain compliance with HIPAA and other regulatory standards, safeguarding patient confidentiality.
- Stay updated on best practices for remote nursing care and telehealth services.
