We are seeking a compassionate Hospice Nurse Practitioner to join our interdisciplinary team. Care is delivered across a variety of settings, including private homes, adult family homes, assisted living communities, and skilled nursing facilities. This position includes weekend responsibilities to ensure essential care and support for patients and families. The NP will focus on responding to urgent or emergency needs, managing symptoms, and promoting the comfort and well-being of patients.

Primary Responsibilities
- Assess and manage hospice patients’ physical, emotional, and spiritual needs.
- Develop and implement individualized care plans in collaboration with the team.
- Prescribe and manage medications and treatments within scope of practice.
- Educate patients and families on symptom management and end-of-life planning.
- Provide clinical guidance to staff and participate in quality improvement initiatives.
- Maintain accurate documentation in compliance with hospice regulations.
- Current, active Washington State NP license in good standing
- Active BLS certification
- Valid driver’s license and reliable transportation
- Minimum 1 year nursing experience in hospice or home health preferred
- Minimum 2-3 years of healthcare related experience required
- Proficient with EMR systems and healthcare technologies
- Ability to exercise sound judgment, initiative, and work independently within scope of practice
- Adaptable to diverse social, cultural, and religious backgrounds
