About This Position
POSITION SUMMARY:
The Outreach and Enrollment Specialist provide a key role in increasing access to health insurance coverage and quality primary, dental, and behavioral health care for both established patients and community residents. This position provides outreach, education, and enrollment assistance to uninsured and underinsured individuals, while cultivating partnerships with local organizations to support community health initiatives.

ESSENTIAL DUTIES & RESPONSIBILITIES:
Enrollment & Patient Support
1. Offer structured education on health coverage options and provide follow-up support to maintain ongoing coverage.
2. Provide one-on-one enrollment assistance for uninsured children and adults in subsidized, low-cost, and free health insurance programs, including the Health Insurance Marketplace, Medicaid, and CHIP.
3. Assist patients with completing applications, gathering necessary documentation, and navigating the enrollment and renewal process.
Outreach & Community Engagement
1. Engage with Lead Patient Service Representative (PSR) to link with all uninsured patients upon registration for appointment.
2. Review Lapse Detection Report Lists and conduct outreach with uninsured or those pending loss of insurance.
3. Attend FPCN community events and partner health fairs to table and promote Outreach & Enrollment services.
Reporting & Documentation
1. Required participation in PA State Pennis assister monthly Meetings & Education sessions
2. Track and report application outcomes, enrollment data, and patient engagement encounters in the Athena (EMR).
3. Complete weekly reporting requirements to PA CHC
EXPERIENCE AND REQUIREMENTS:
Candidates should possess a diverse range of skills and experience including but not limited to:
Required Education and Experience
- Bachelor’s degree in a related field preferred, or equivalent relevant experience.
- Licenses and Professional Certifications (or ability to obtain)
- At least one year of experience with Pennsylvania health insurance programs, public benefits, and community resources is a plus.
- Ability to travel locally within the service area using personal or public transportation (mileage reimbursed).
- Ability to maintain confidentiality and handle sensitive information with professionalism.

Other Qualifications
- Ability to pass required background screenings. (Non-clinical) OR Ability to pass required background screenings and credentialing verifications.
- Ability to complete and pass the PA State Pennie Assister Training and obtain Assister License certification ID.
- Maintains Annual re-certification of PA State Assisters License.
- Required TB/PPD Test.
- Optional Immunizations: Hep B Vaccination and Flu Vaccination.
- Current valid CPR Certification required.
- Maintain current, valid, applicable position credentials.
- Maintain reliable transportation to travel throughout the network and/or participate in events outside the network as needed. (all staff if applicable)
- Must be willing to work a minimum of 40 hrs. weekly (Hybrid)
Lifting Requirements
LIGHT – Exerting up to 20 pounds of force frequently, and/or negligible amount of force constantly to move objects. The use of arm and/or leg control requires force greater than sedentary, but worker still sits majority of time.
