Position Summary
The Case Manager supports youth at high risk of violence involvement by providing culturally responsive, trauma-informed case management services. This role focuses on building trust, connecting youth to resources, and supporting their progress toward personal, educational, and career goals.

Qualifications:
- Bachelor’s degree in Social Work or a related field
- Valid driver’s license and access to a reliable vehicle
- Strong commitment to working with youth and seeing tasks through to completion
- Excellent communication, outreach, and relationship-building skills
- Provide individualized, youth-centered case management and goal planning
- Build trusting relationships and engage youth through proactive outreach strategies
- Conduct intake, screening, and assessments to determine eligibility and service needs
- Develop and implement case plans using a strength-based, healing-centered approach
- Connect youth to services, including education, employment, housing, and mental health resources
- Transport youth to appointments and provide transportation support as needed
- Attend court appointments and assist with advocacy and support services
- Maintain accurate documentation and report weekly progress in designated data systems
- Mental health and social-emotional support (including CBT-informed activities)
- Education (school re-engagement, GED, post-secondary enrollment)
- Employment and job readiness programs
- Health and wellness services
- Housing and basic needs support
- Access to public benefits and other supportive services
Engage and recruit youth through community outreach and referrals from schools, courts, foster care systems, shelters, law enforcement, and community-based organizations. Collaborate with partners to ensure coordinated, youth-driven services.
Working Conditions
- Field-based work, including travel within the community
- OSHA Category 3: No regular exposure to blood or bodily fluids
