About This Position
The Coordinated Specialty Care (CSC) program will work with individuals who are between the ages of 15-30 years of age and are in the early stages of a primary psychotic disorder.
The CSC case manager, under the supervision of the CSC Team Leader, will work with individuals and families of adolescents and young adults with severe emotional disturbance (SED) or severe and persistent mental illness (SPMI) using the RAISE model of care. Facilitating a team planning process that is strengths based, focused on underlying needs, and determined by the individuals and families of those enrolled. The focus of this position is providing Intensive Services to a caseload of up to 30 individuals and their families enrolled in the CSC program, adhering to agency productivity standards. The CSC Case Manager is also responsible for developing and organizing stakeholder meetings and participating in outreach efforts. A flexible schedule is required to meet the needs of the enrollees served. The CSC Case Manager needs a good working knowledge of available community resources and must be able to effectively build working relationships with those resources. Extensive travel is required.

MINIMUM JOB QUALIFICATIONS:
- Bachelor’s degree from an accredited college or university with a major (or the equivalent to a major) in psychology, social work, medicine, nursing rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood intervention, or a registered nurse.
PREFERRED JOB QUALIFICATIONS:
- Demonstrate ability to read, write, and speak English and Spanish.
- Work experience in a health care/clinic environment.
- Familiarity with psychiatric and mental health related terminology to include crisis response.
KNOWLEDGE, SKILLS, AND ABILITIES:
- Strong understanding of mental health diagnoses; treatment modalities-, and recovery-oriented practices-.
- Ability to conduct mental health assessments and interpret clinical documentation.
- Skilled in crisis intervention, de-escalation techniques, and evidence-based rehabilitative practices.
- Excellent written and verbal communication skills, including report writing and presenting in court settings.
- Proficiency in using electronic health records (EHRs) and secure documentation systems.
- Ability to work independently and collaboratively within multidisciplinary teams. .
- Strong organizational and time management skills, with attention to detail and confidentiality.
- Maintain accurate and timely case notes, reports, and documentation for court and agency records.
- Ability to work cooperatively and effectively with other professionals, individuals, and groups.
- Ability to exercise tact, show initiative, and use good judgment in dealing with people even in crisis.
- Ability to develop productive and outcome focused relationships with individuals and family members.
- Ability to identify problems/gaps in service:
- Ability to advocate effectively for an individual.
