Intake and documentation of referrals for all branches utilizing the Meditech system.
- Intake and documentation of orders for all branches utilizing the Meditech system.
- Promote the delivery of appropriate home health services by acting as a resource for the public. Assist in identifying resources and accurately explaining agency policies, procedures, and third party pay source coverage/reimbursement guidelines.
- Coordinate all services ordered.
- Demonstrate effective communication skills with all internal and external customers. Assist Intake Director with Quality Assurance and supervision of LPN Intake staff.

Requires RN with current state licensure and successful completion of nursing education from an approved school of nursing.
Prefer home health experience or professional nursing experiences that encompass a variety of clinical settings.
Must possess excellent interpersonal and oral communication skills, especially listening and telephone skills.
Must have broad nursing knowledge base.
Must have working knowledge of available community resources and alternatives to home care. Must possess considerable knowledge of home care guidelines and be able to read and interpret guidelines, policies, and procedures.
Basic computer skills and the ability to use a keyboard.
CPR certification and completion of a minimum of 16 hours continuing education annually required
Background checks/drug-free workplace.
