We are looking to hire a Community Case Manager for Adults with Development Disabilities and Autism to our program and prefer those with experience. This is a hybrid position. Our case manager will work out of our Bangor office initially for training and full support. After that, the position allows for flexibility and creation of your own schedule to see clients in Bangor- Rockport-Belfast area.
We work as a team to support each other and promote self-care, as well as care for those we support, utilizing a Trauma Informed approach. Supervision and support is available to promote employee success and wellbeing,
As a Case Manager you will be expected to:
1. Work out of our office for the few months or as needed, depending on your experience, while receiving/completing training, developing skills, receiving needed support and growing your case load. Once able to demonstrate the necessary skills and abilities, remote remote work will be available and you can create your own schedule
2. Complete both State of Maine and Brighter Heights Maine required trainings;
3. Provide own transportation to/from meetings, home and community visits (must have vehicle), provide proof of license, insurance and registration. Mileage reimbursement is provided;
4. Oversee, monitor, coordinate all needed services for individuals on your case load;
5. Communicate effectively with individuals and their teams;
6. Make referrals, coordinate services and obtain funding for needed services;
7. Develop, monitor, report and assure annual Person Centered Plans address all needs and desires of the individuals you support;
8. Document effectively and within a timely manner in the State of Maine Evergreen System;
9. Manage a caseload of 20-23 individuals;
10. Meet billing expectations weekly;
11. Participate in monthly supervisions;
12. Communicate with supervisor as needed for support, problem solving, help with understanding processes/procedures, etc.;
13. Maintain professional and ethical boundaries.

Requirements:
- strong time management, organizational skills and dedicated
- excellent verbal and written communication skills
- experience in working with people with developmental disabilities or a related human service field
- 2 years of prior case management experience preferred
- strong computer skills, ability to learn an EMR
- Bachelor’s degree from an accredited institution
