About Bremen Hospital
Community Hospital of Bremen is a 24-bed critical access hospital serving Bremen, Indiana, and the surrounding communities in Marshall and St. Joseph counties. As part of Beacon Health System, patients have access to providers, resources and services from across the entire health system. The hospital provides the community with: Emergency Department; diagnostic imaging and lab; radiology, including MRI, CT and ultrasound; general surgery; obstetrics; sleep lab; mammography; physical and occupational therapy; occupational health; swing bed services; and infusion therapy.

What You’ll Do

As a Patient Access Rep, you'll be the first point of contact for our patients. In this vital role, you'll deliver exceptional service while accurately registering patients for care. You'll verify insurance eligibility, collect co-pays, and ensure all necessary documentation is completed—all while upholding our mission to deliver outstanding care and connect with heart.

Patient Access Rep Job Responsibilities

Patient Registration & Financial Intake

  • Greet and register patients for admissions, transfers, and outpatient procedures.
  • Collect and enter demographic, insurance, and physician data into electronic systems (STAR, PHS, Cerner).
  • Verify insurance eligibility, benefits, and collect applicable payments; reconcile cash drawer.
  • Obtain required consent and assignment of benefits signatures.
  • Explain pre-certification and insurance requirements; assist with MSP and ABN forms.

Insurance Verification & Pre-Certification

  • Audit insurance eligibility and confirm network participation for transfers and scheduled services.
  • Coordinate with insurance companies and physician offices for pre-certifications and authorizations.
  • Enter authorization numbers and benefits accurately to support timely claims.

Clerical & Coordination

  • Prepare ID bands, charts, and escort arrangements.
  • Handle phone/fax inquiries related to registration.
  • Manage billing, documentation, and resolve insurance issues.
  • Refer patients with outstanding balances to Financial Counselors or Eligibility Specialists.

What You Bring

As a Patient Care Rep, this role requires proficiency in office and keyboarding skills, use of reference materials, effective telephone and office equipment operation, and strong interpersonal skills to interact professionally and courteously with patients from diverse backgrounds. It demands verbal communication skills for collaboration with patients, the public, physicians, and other departments, strict adherence to Community Hospital of Bremen (CHB) confidentiality policies, sound judgment, composure in stressful situations, and the ability to multitask and work independently. The employee must remain knowledgeable of and comply with all applicable federal and state laws, regulations, and hospital policies, uphold ethical behavior and standards of conduct, protect patient information, and complete annual orientation or intranet-based training with a quiz submission to Human Resources.

Required Qualifications

  • The knowledge, skills and abilities as indicated below are acquired through the successful completion of a high school diploma or equivalent is preferred.
  • Must be a minimum of 17 years of age.
  • A minimum of one year of previous clerical/computer experience is preferred.
  • Open to nursing students
