Position Summary
The Occupational Therapist (OT) provides skilled occupational therapy services to patients in their homes in accordance with physician orders, the plan of care, agency policies, and applicable state and federal regulations.
The OT evaluates patient functional status, develops and implements therapy interventions, and works collaboratively with the interdisciplinary care team to improve patient independence, safety, and quality of life.
Reporting Structure
This position reports to the Director of Nursing (DON), Clinical Manager, or designated clinical leadership. The OT functions as part of the interdisciplinary care team and provides supervision to the Occupational Therapy Assistant (OTA) and Home Health Aide when required by the patient’s plan of care and agency policy.
Scope of Practice
The Occupational Therapist performs services within the scope of Florida OT licensure, Medicare Conditions of Participation, physician orders, and agency policies. The OT is responsible for evaluation, treatment planning, reassessment, and therapy supervision within the home health setting.
Essential Job Functions
1. Patient Evaluation & Care Planning
Perform initial and ongoing patient evaluations to assess functional status and therapy needsComplete comprehensive assessments and participate in OASIS-related coordination as applicableDevelop, implement, and update individualized occupational therapy treatment plansEnsure physician orders are appropriate and aligned with the patient’s therapy needs2. Skilled Therapy Services
Provide skilled occupational therapy interventions in accordance with physician orders and established care plansGuide patients in therapeutic exercises, self-care activities, and functional mobility related to daily livingInstruct patients in the safe use of wheelchairs, braces, splints, prosthetic, and orthotic devicesSupport patient safety, independence, and restoration of functional abilities in the home setting3. Patient & Family Education
Teach and counsel patients and caregivers regarding therapy goals, home safety, adaptive techniques, and self-care strategiesEvaluate the home environment and identify modifications or community resources needed to support careCoordinate outpatient services when procedures cannot be safely performed in the home4. Documentation & Compliance
Complete timely and accurate clinical documentation, progress notes, reassessments, and discharge summariesEvaluate effectiveness of therapy interventions and update the plan of care as neededMaintain compliance with Medicare requirements, state regulations, and agency policies5. Team Collaboration & Supervision
Communicate regularly with physicians, nurses, and interdisciplinary team members regarding patient progress and needsSupervise the Occupational Therapy Assistant (OTA) in accordance with agency policy and regulatory requirementsSupervise the Home Health Aide when Occupational Therapy is the only skilled discipline involved in patient careParticipate in care conferences, discharge planning, staff meetings, and required in-servicesQualifications
Graduate of an accredited Occupational Therapy program required. Current state Occupational Therapist (OTR) license in good standing required. Minimum of one (1) year of experience in occupational therapy required; home health, acute care, or rehabilitation experience preferred.Current CPR certification requiredValid driver’s license, reliable transportation, and auto insuranceAbility to pass background screening and all compliance requirementsSkills We Value

Frequent travel to patient homes within assigned territoryAbility to stand, walk, bend, lift, and assist patients as neededAbility to perform therapy assessments and interventions in home environmentsAbility to use electronic documentation systems and therapy equipmentAbility to communicate effectively with patients, families, and care team membersWork is performed in patient homes and community settings with exposure to variable environmental conditions, including pets, smoke, infectious diseases, and weather-related travel conditions.
