Position Overview Seeking an experienced Speech-Language Pathologist (SLP) to deliver in-home speech therapy services. Work independently while collaborating with a multidisciplinary care team.

Responsibilities

Evaluate speech, language, cognition, voice, and swallowing

Develop and implement treatment plans

Provide skilled home health speech therapy

Educate patients and caregivers

Document care and patient progress

Maintain Medicare, HIPAA, and compliance standards

Qualifications

Graduate of accredited SLP program

Active state SLP license

2+ years clinical experience (home health/SNF preferred)

OASIS and EMR experience preferred (WellSky/Kinnser a plus)

Valid driver’s license and reliable transportation
