About This Position

Description:

Do you have the heart to serve? Do you know someone who is needing a Direct Support Professional (DSP) to provide services including IHS (individualized home supports), ICLS (individual community living supports), or Respite care whom you are interested in working with? Whether the individual served is new, or is already associated with our agency at Alpine Home Health, we can look into having you work as a DSP. As a Direct Support Professional, you will be responsible for providing support and assistance to individuals with disabilities in accordance with the 245D regulations.

Responsibilities:

- Provide direct care and support to individuals with disabilities in a respectful and compassionate manner

- Implement individualized care plans and behavior support plans as established by the care team

- Accompany and drive individuals to appointments, community activities, and run errands

- Maintain accurate and timely documentation of services provided

- Ability to use the Electronic Verification Visit App to log hours and services

Make sure to specify IHS w/ Training (individual, family, or both kinds of training)

-Train the individual you support skills he or she needs to live more independently

-Train the family of the individual you support skills they need to assist the individual you are serving

Requirements:

- Valid driver's license and reliable transportation (not required for night supervision)

- Ability to work flexible hours, including weekends and holidays

- Excellent communication and interpersonal skills

- Ability to work independently and as part of a team

- Ability to pass a MN Dept of Human Services background check

- Have not been excluded from participating in Federal or State Healthcare Programs

- Ability to complete Orientation and Positive Support Rule Training online or a combination of in-person and online

-Required Vulnerable Adult Training through the Department of Human Service

-Must be able to stand, walk, bend, and stoop down to clean and to care for individuals served

-Must be able to exert up to 40 pounds of force to move objects

-An open mind and a caring demeanor

Preferred Qualifications:

- Experience working with individuals with disabilities or vulnerable adults

- Knowledge of 245D Home and Community Based Service Standards, prior experience in caregiving or working as a Direct Support Professional, or willing to learn policies prior to assuming responsibilities

- CPR or First Aid training or willing to learn prior to assuming responsibilities
