Position Overview:
We are currently seeking a dedicated and patient-focused Physical Therapist to join our home health care team in Fredericksburg, VA. As a Physical Therapist, you’ll have the unique opportunity to provide one-on-one care in patients’ homes, creating personalized therapy plans that help them regain mobility, independence, and quality of life.

Key Responsibilities:

• Conduct in-home physical therapy evaluations to assess patients’ needs and abilities

• Develop individualized treatment plans in coordination with physicians and interdisciplinary teams

• Provide therapeutic interventions to improve strength, mobility, and functional independence

• Educate patients and caregivers on home exercises and injury prevention techniques

• Maintain accurate and timely documentation within EMR systems

• Monitor progress, adjust care plans as necessary, and ensure continuity of care

• Collaborate with clinical team members to support overall patient well-being

Qualifications:

• Active Physical Therapist license in the Commonwealth of Virginia

• Graduate of an accredited Physical Therapy program

• Preferred: At least 1–2 years of home health or outpatient therapy experience

• Strong communication and organizational skills

• Ability to work independently in a home-based setting

• Reliable transportation and valid driver's license
