Summary:

Provides nursing care assigned by and under the direction of a supervising registered nurse.

Essential Duties and Responsibilities: (Note: Other Duties May Be Assigned)

• Provides nursing care as established on the physician’s plan of care and home health certification.

• Shall provide nursing care always under the supervision of a Registered Nurse.

• Prepare and record clinical notes for the clinical record.

• Provide care to the patient including the administration of treatments and medications.

• Report any changes in the patient’s condition to the registered nurse or to the physician with the reports documented in the clinical record.

• Participates in all required case conferences, utilization reviews, orientations, in-services and continuing education meetings.

Qualifications:

• Graduate of an Accredited School of Practical Nursing
