As a locally owned, mission-driven home health and infusion pharmacy serving the greater Phoenix area, we bring clinical excellence directly to the patients who need it most — and we're looking for an experienced Home Infusion Pharmacist to help us do it.

No problem — we'll train you. What matters most is your clinical judgment, your attention to detail, and your commitment to patient care.
Pay & Benefits:
- Salary: $130K–$160K/yr DOE
- $7,500 Sign-On Bonus (paid $3,750 at start date, $3,750 at 90 days)

Schedule:
- Full-Time, Monday–Friday
- 2 openings — one 1st shift (9:30am–6pm), one 2nd shift (10:30am–7pm)
- On-call rotation with a required phone response time of within 30 minutes
- Holiday rotation as scheduled
Key Responsibilities:
- Perform pharmacy tasks including pharmaceutical calculations, data entry, and general workflow management
- Oversee and verify sterile and non-sterile compounding performed by pharmacy technicians in compliance with USP 797 and USP 800 standards (training provided for pharmacists without hands-on compounding background)
- Review and verify medication orders for accuracy, drug interactions, contraindications, and dosing
- Monitor patient drug therapy, interpret lab data, and adjust care plans to optimize outcomes
- Collaborate with physicians, nurses, and interdisciplinary care teams to coordinate patient care
- Educate patients and caregivers on medication use, safe administration, and what to expect
- Supervise pharmacy technicians and compounding personnel
- Use effective strategies to achieve daily goals, including prioritization, delegation, deferral, and reassignment of duties
- Demonstrate proficiency with pharmacy software and data entry systems
- Ensure full compliance with HIPAA, OSHA, state Board of Pharmacy, and accrediting body standards
- Participate in QAPI initiatives and support a culture of continuous improvement
- Perform other duties as assigned
Required:
- Doctor of Pharmacy (PharmD) or B.S. in Pharmacy
- Active, unrestricted Arizona Pharmacist license
- 2+ years of pharmacist experience
- Proficiency with pharmacy software and data entry systems
- Strong organizational and time-management skills with the ability to prioritize effectively in a fast-paced environment
- Strong communication and patient-facing skills
- Ability to participate in on-call rotation with a 30-minute phone response time and holiday rotation as scheduled
Preferred:
- 1+ year of experience in a home infusion, specialty pharmacy, or sterile compounding setting
- Hands-on compounding experience, including sterile and non-sterile compounding in USP 797 and USP 800 compliant environments
- BCSCP certification
- Experience with IV antibiotics, TPN, or specialty biologics in a home infusion setting
- Experience supervising or directing pharmacy technicians
