The HOPE Project Support Staff will promote recovery by assisting individuals to identify and overcome barriers to recovery, develop recovery capital, and serve as a recovery guide and mentor for those seeking and sustaining recovery. The HOPE Project Support Staff will assist in the process of wellness planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive needs in recovery.
- Assists in the screening process, advocating for candidates meeting admission criteria.
- Serves as a client mentor to plan and coordinate a package of health and social services that is individualized to meet a client’s needs in recovery.
- Assists clients with identifying and removing barriers to long-term recovery goals.
- Monitors, tracks, and providers support to a client, throughout the course of their treatment and HOPE program.
- Assists the client in strengths-based problem solving while they are re-establishing external resources.
- Connects clients with recovery support services in their communities.
- Encourages hope, optimism, and healthy living.
- Advocates with and enhances outside external resources to enhance existing client success in recovery and overall wellness.
- Participates in discharge planning and transitional needs throughout the HOPE program.
- Follow up of clients to monitor engagement in ongoing care and monitor for relapse potential.
- Complete all required documentation in electronic health record and as required to be in compliance with HOPE Project funding.
- Accepts, follows, and sets an example that represents The Walker Center’s policies, philosophy, mission and values.
- Demonstrates excellent service in all interactions to all those we come into contact with (coworkers, clients, families, venders, general public).
- Adheres to an established work schedule.
- Maintains confidentiality and exercises good judgement about what to say and when to say it.
- Is a self-starter, exhibits initiative necessary to complete tasks.
- Possesses ability to prioritize tasks to ensure completion according to significance.
- Able to work calmly and handle stressful situations which occur during busy periods of time.
- Consistent in checking accuracy of own work; keeps errors to a minimum and accepts personal responsibility for work.
- Contributes to a positive work environment through interactions with others.
- High School Diploma or GED required; additional education preferred such as ADC, SUDA, LSW.
- Proficient in Microsoft Outlook and general computer systems, with the ability to learn and maintain Electronic Medical Records (EMR).
- Ability to type and perform repetitive motions related to computer and documentation tasks.
- Must be at least 21 years of age.
- Must possess a valid driver's license and maintain a good driving record.
- If in recovery from a Substance Use Disorder, must have a minimum of two (2) years of sobriety from substances of use.
- Ability to perform the physical requirements of the position including walking, standing, bending, stooping, pushing, pulling, lifting, typing, and repetitive motions.
- Must obtain CPR certification within ninety (90) days of hire.
- Must pass pre-hire requirements including a criminal background check, or Idaho Department of Health and Welfare Background Check Waiver for Recovery Coach, urinalysis (UA), and TB test.
If hired through the Background Check Waiver for Recovery Coach process, the employee shall comply with all supervision, continuing education, training, and certification requirements by the IBADCC.
