The Latin American Health Alliance (LAHA) is a nonprofit organization dedicated to improving the health and well-being of underserved communities across Central Massachusetts. Through culturally responsive addiction recovery services, healthcare access, and educational support, we empower individuals to build healthier, more stable lives.

We are seeking a Residential Program Case Manager to join our team at Hector Reyes House, our residential recovery program for Latino men. This role is ideal for someone who is passionate about recovery, advocacy, case management, and helping individuals achieve long-term success in their recovery journey.

Position Overview

As a Residential Program Case Manager, you will work closely with residents, clinicians, medical providers, and community partners to provide individualized support and guide residents toward achieving their treatment, recovery, and life goals.

This position requires compassion, professionalism, strong organizational skills, and a commitment to trauma-informed, person-centered care.

Key ResponsibilitiesCase Management & Resident Support

Conduct weekly individual sessions with residents and document progress.
Develop and monitor individualized treatment and recovery plans.
Coordinate discharge planning and aftercare services.
Assist residents with applications for housing, employment, benefits, and community services.
Advocate for residents and connect them with appropriate community resources.
Group Facilitation & Recovery Services

Facilitate educational and recovery-focused group sessions.
Support residents in achieving personal, vocational, and recovery goals.
Promote recovery-oriented practices and support diverse pathways to recovery, including Medication-Assisted Treatment (MAT).
Collaboration & Clinical Support

Work as part of a multidisciplinary team including clinical and medical professionals.
Participate in treatment planning and case consultation meetings.
Communicate resident progress, concerns, and needs effectively to team members.
Residential Program Operations

Assist with admissions, discharges, drug testing, transportation, and medication-related procedures.
Conduct room inspections and support household management activities.
Ensure compliance with program policies, procedures, and safety standards.
Respond appropriately to crises and emergency situations.
Preferred Qualifications

Experience in case management, behavioral health, addiction treatment, social services, or a related field.
Understanding of trauma-informed care and recovery-oriented services.
Experience working with individuals affected by substance use disorders.
Desired Skills

Strong communication, documentation, and organizational abilities.
Ability to build rapport with diverse populations.
Excellent problem-solving and crisis intervention skills.
Ability to work independently and as part of a team.
Commitment to resident-centered, non-judgmental care.
Additional Preferences

Bilingual English/Spanish strongly preferred.
Experience working with Latino communities is a plus.
