Wage Range: $134,326 - $214,926 per year

Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications or degrees. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.

Our Team
As an interdisciplinary team we work together to enrich the health and wellbeing of every life we touch. Our care teams are composed of the following:

  • Physicians-The patients Attending Physician continues to provide oversight of care and Hospice Physicians consult with hospice staff and the attending physician.
  • Nurses (RN or LPN)
  • Social Workers
  • Certified Nursing Assistants (Home Health Aides)
  • Spiritual Care (Chaplains)
  • Pharmacists
  • Trained Volunteers- offer companionship for patients and respite for families


Job Summary
Role will manage a multi-disciplinary team in a designated geographical area comprised of RN's, LPN's, Social Workers and Home Health Aide/CNAs. Role will assume 24-hour accountability and provision of quality of care and service for patients at end of life. Responsible for planning, developing, organizing, integrating, and overseeing programs, staff, and budget in support of the organizational goals and commitments.
Primary Duties:

  1. Responsible for service line(s) to include, but not limited to, Medical and Psychiatric nursing, rehabilitative and social work services.
  2. Ensures that operations follow established policies and procedures and comply with all applicable standards and regulations to include, but not limited to: licensure, Medicare Conditions of Participation, JC, OSHA regulations and accepted standards of practice. Ensures, promotes, and assesses compliance with regulatory agency standards and acts as resource to staff to answer regulation based questions.
  3. Facilitates patient care and operations including organization and management within established policies, procedures, standards of care, budgetary parameters and strategic objectives.
  4. Performs other duties as assigned.

License, Certification, Education or Experience:

REQUIREDfor the position:
  • Master's Degree in Social Work (MSW)
  • One of the following credentials is required: Licensed Independent Clinical Social Worker (LICSW) or Licensed Advanced Social Worker (LASW)
  • Current Healthcare Provider BLS certification by date of hire
  • An understanding of financial management, budgeting skills, and a commitment to the provision of cost-effective care
  • Knowledge of Federal, State, and JC regulations and requirements
  • Knowledge of continuous quality improvement, and leadership process knowledge and skills
  • Effective customer relationship skills, interest and willingness to create and maintain relationships with key customers, particularly physicians and payers
  • Ability to assess, plan, implement and evaluate patient care
  • Demonstrate ability to communicate clinical/education knowledge to staff, patients, and families
  • Demonstrate or acquire skills to complete clinical skills/competency assessment checklist
  • Verbal and written language skills, visual and aural skills in order to communicate and ensure and deliver safe patient care, and to provide a legal record of this care
  • Current Washington State Drivers license and proof of insurability


DESIREDfor the position:

  • Bachelors, Masters or Doctoral degree in Health care field that licensure is in.
  • 2 years of Home Health and/ or Hospice experience
  • Supervisory experience
