Job Description
The NeuroMedical Center is currently seeking an Advanced Practice Provider (APP) to join our Neurology Department!
As Louisiana’s leading neuroscience center, The NeuroMedical Center offers the opportunity to work closely with Neuroradiologists, Neurosurgeons, Neurologists, and a highly skilled clinical team focused on diagnosing and treating neurological conditions.
The APP will provide comprehensive primary care services, including new and established patient visits, cognitive assessments and care planning, annual wellness visits, and other appropriate clinical services. Prior experience providing care to older adults or individuals living with dementia is preferred.
Responsibilities
- Assists in the evaluation, diagnosis, and treatment of patients.
- Accurately and timely documents patient care in the medical record.
- Conducts patient interviews and obtains and documents medical, psychosocial, and health histories, including current conditions and potential health concerns.
- Collaborates with physicians in diagnosing medical conditions, developing individualized care plans, and determining appropriate treatment.
- Provides patient and family education regarding diagnoses, treatment plans, medications, and ongoing care needs.
- Coordinates with physicians and other members of the healthcare team to support continuity and quality of patient care.
- Ideal candidate will have a minimum of 2 years’ experience providing dementia sensitive primary care.
- Completion of an accredited Nurse Practitioner program or Physician Assistant program.
- Licensed as appropriate in the state of Louisiana.
- American Heart Association BLS and Healthcare Provider certifications required.
