Job Summary

The Clinical Manager is responsible for providing clinical, operational, and supervisory oversight of residential services and ensuring that participants receive safe, person-centered, high-quality care. The Clinical Manager works closely with residential managers, Direct Support Professionals (DSPs), nurses, healthcare providers, behavioral health professionals, families/guardians, case managers/CCS, and other members of the interdisciplinary team.

The Clinical Manager provides clinical oversight of participants, supports managers in addressing health and behavioral concerns, monitors compliance with Individual Plans/Person-Centered Plans (PCPs) and Behavior Support Plans, oversees medication management and clinical documentation, investigates incidents and deficiencies, and ensures that residential operations remain compliant with agency policies and applicable Maryland DDA, nursing, licensing, and regulatory requirements.

Minimum educational/licensure requirement:

The candidate must hold either a current Registered Nurse (RN) license or a degree in Social Work (SW) license from an accredited institution. Additional experience in residential services, developmental disabilities, behavioral health, or clinical management is strongly preferred.

Key Responsibilities

Clinical Oversight of Participants

  • Provide clinical oversight for all residential participants, including review of diagnoses, medical history, allergies, medications, health risks, hospitalizations, falls, seizures, behavioral/medical changes, and other significant clinical concerns.
  • Identify participants with increased medical or behavioral risk and maintain appropriate high-risk tracking and monitoring systems.
  • Develop, coordinate, and monitor individualized clinical protocols for participants with complex needs, including diabetes, seizure disorders, aspiration risk, dysphagia, constipation, hypertension, wounds, fall risk, behavioral health conditions, and other medical conditions.
  • Monitor changes in participants' conditions and ensure appropriate escalation to nursing, physicians, emergency services, or other healthcare providers.
  • Ensure staff understand and appropriately implement participant-specific medical and clinical protocols.
  • Serve as a clinical resource to residential managers and staff regarding participant health and safety concerns.

Admissions, Transfers, and Discharges

  • Conduct interviews and participate in the assessment of prospective residential participants.
  • Complete or coordinate clinical reviews for potential admissions to determine whether the agency has the appropriate staffing, training, equipment, environmental supports, and clinical resources necessary to safely support the individual.
  • Oversee the clinical components of admissions, transfers, hospital returns, and discharges.
  • Coordinate with appropriate departments and members of the interdisciplinary team throughout the admission and transition process.
  • Oversee participant discharges and ensure all personal belongings and required property documentation are properly accounted for and signed out.
  • Participate in determining appropriate transition and discharge plans based on participant needs.

PCP and Behavior Support Plan Oversight

  • Attend participant PCP meetings and ensure staff are trained regarding applicable PCP requirements.
  • Attend participant Behavior Support Plan/behavioral meetings and ensure staff understand and follow approved procedures.
  • Coordinate and conduct monthly PCP and behavioral staff training, as required.
  • Obtain and maintain required staff signatures verifying training and understanding.
  • Monitor staff compliance with PCPs, Behavior Support Plans, medical protocols, documentation requirements, and participant-specific procedures.
  • Review behavioral and clinical documentation to ensure services are being delivered and documented appropriately.
  • Implement or coordinate evidence-based behavior-management strategies designed to promote positive behaviors and reduce challenging behaviors, consistent with approved plans.
  • Schedule and attend behavior plan meetings and ensure resulting recommendations are communicated to appropriate staff.

Medication Management and Clinical Safety

  • Oversee medication administration systems, Medication Administration Records (MARs), medication errors, missed doses, pharmacy concerns, medication changes, PRN medications, and medication-related corrective actions.
  • Conduct routine medication and medication-storage audits.
  • Investigate medication errors and identify corrective and preventive actions.
  • Monitor recurring medication deficiencies and develop strategies to prevent recurrence.
  • Ensure medication changes resulting from physician appointments, hospitalizations, or other healthcare encounters are accurately documented and implemented.
  • Ensure physician orders for new, changed, or discontinued medications are obtained, communicated, documented, and implemented appropriately.
  • Monitor PRN effectiveness and ensure appropriate documentation and follow-up.
  • Ensure DSPs receive required medication-related training and demonstrate competency as applicable to their assigned responsibilities.

Healthcare Appointments and Hospital Follow-Up

  • Ensure PCP, specialist, dental, psychiatric, therapy, laboratory, and other healthcare appointments are scheduled, attended, and appropriately documented.
  • Monitor recommendations resulting from medical appointments and ensure follow-up actions are completed.
  • Review emergency department visits, hospitalizations, and discharge instructions.
  • Ensure hospital discharge recommendations, medication changes, follow-up appointments, and other clinical instructions are implemented.
  • Communicate with physicians, specialists, pharmacies, nurses, therapists, behavioral health providers, and other healthcare professionals as appropriate.

Clinical Documentation and Chart Audits

  • Conduct routine clinical documentation audits to ensure records are complete, accurate, timely, and compliant.
  • Review medical records, progress notes, MARs/TARs, health records, appointment documentation, incident reports, nursing documentation, physician orders, and clinical follow-up.
  • Ensure required medical tasks are completed and appropriately signed.
  • Monitor care notes, CDS notes, client logs, and health-monitoring documentation, including blood pressure, weight, blood sugar, seizure activity, bowel movements, menstrual/period logs, and other required health information.
  • Maintain participant records in a continuous state of audit and survey readiness.
  • Identify missing, inaccurate, or incomplete documentation and ensure corrective action is taken.

Incident Investigation and Risk Management

  • Clinically review medication errors, injuries, falls, hospitalizations, choking/aspiration events, seizures, unexplained changes in condition, and other health-related incidents.
  • Investigate departmental deficiencies and clinical concerns.
  • Conduct or coordinate root-cause analysis for recurring incidents and develop corrective/preventive action plans.
  • Identify trends and patterns that may place participants at increased risk.
  • Ensure managers and staff understand appropriate incident reporting and escalation procedures.
  • Monitor compliance with required reporting and follow-up procedures.

Staff Supervision, Training, and Competency

  • Provide clinical supervision and guidance to residential managers while managers maintain direct supervision of DSPs.
  • Ensure managers appropriately address health-related concerns with their teams.
  • Oversee new employees and transfer employee orientation and training from start to completion.
  • Coordinate staff training across departments and facilitate agency-required training.
  • Conduct or coordinate staff training related to PCPs, Behavior Support Plans, medical conditions, medication procedures, documentation, emergency procedures, changes in condition, and escalation requirements.
  • Train managers and staff on when to contact nursing/medical providers, emergency services, or 911.
  • Ensure staff demonstrate competency in medication administration and individual-specific clinical procedures when applicable.
  • Monitor staff compliance with required training and maintain appropriate training records.

Staffing and Residential Operations

  • Process and oversee residential staff scheduling to ensure adequate staffing and appropriate coverage.
  • Address day-to-day staffing, participants, and residential operational concerns.
  • Provide departmental and staff oversight.
  • Ensure appropriate response to staffing shortages, call-outs, and other coverage concerns.
  • Monitor that staff appropriately report clinical concerns to the on-call nurse or designated clinical escalation resource.
  • Identify departmental gaps and develop practical solutions in collaboration with leadership.
  • Support residential managers in resolving operational and participant-care concerns.

Performance Management and Quality Improvement

  • Oversee departmental performance improvement activities.
  • Conduct and/or coordinate employee performance evaluations.
  • Monitor employee performance and identify training, coaching, or corrective-action needs.
  • Establish and monitor measurable clinical and operational Key Performance Indicators (KPIs).
  • Track indicators such as:
  • Medication-error rates
  • Missed medications
  • Hospitalizations and ER utilization
  • Falls
  • Seizures and other significant clinical incidents
  • Overdue appointments
  • Overdue laboratory work
  • Incident trends
  • Documentation deficiencies
  • Training compliance
  • PCP/Behavior Support Plan compliance
  • Complete required departmental indicators through the agency's Dash Report monthly.
  • Develop corrective and preventive action plans based on identified trends and deficiencies.

Quality Assurance and Regulatory Compliance

  • Oversee departmental audits and address findings identified through Quality Assurance (QA).
  • Ensure corrective actions are implemented and monitored to completion.
  • Maintain department records and clinical documentation in a continuous state of survey/audit readiness.
  • Help ensure compliance with applicable Maryland Developmental Disabilities Administration (DDA) requirements, nursing requirements, agency policies, licensing standards, and other applicable regulatory requirements.
  • Monitor staff compliance with participant care plans, clinical protocols, documentation requirements, and agency procedures.
  • Collaborate with QA and leadership to identify systemic deficiencies and sustainable solutions.

Emergency Preparedness and Safety

  • Oversee monthly unit fire drills and semi-annual disaster drills.
  • Ensure drills are properly conducted, documented, and reviewed.
  • Monitor staff understanding of emergency procedures and participant-specific emergency needs.
  • Ensure emergency preparedness plans account for participants' medical, behavioral, mobility, and other support needs.

Participant Rights and Community Support

  • Attend legal proceedings involving participants when required and appropriate.
  • Ensure participant allowances are distributed weekly in accordance with agency policies and applicable requirements.
  • Promote participant dignity, rights, choice, independence, safety, and person-centered care.
  • Ensure staff interactions and services reflect the participant's individual needs and approved plans.

Interdisciplinary Communication

  • Maintain ongoing communication with:
  • Primary care providers and specialists
  • NursesPharmacies
  • Behavioral health providers
  • Therapists
  • Case managers/CCS
  • Families and guardians
  • Residential managers
  • DSPs
  • QA and administrative departments
  • Other members of the participant's interdisciplinary team
  • Coordinate information across departments to ensure continuity of care and timely resolution of participant needs.
  • Ensure significant clinical concerns are communicated to the appropriate parties promptly.

On-Call Clinical Escalation

  • Serve as the designated clinical escalation point, when assigned, or establish and maintain an appropriate nursing escalation process for residential managers.
  • Provide guidance regarding significant changes in participant condition, medication concerns, injuries, falls, seizures, behavioral/medical changes, and other urgent clinical matters.
  • Ensure managers understand appropriate escalation procedures and documentation requirements.
  • Ensure staff know when emergency medical services or 911 should be contacted.

Qualifications Requirements

  • Must possess one of the following:
  • Current, valid Registered Nurse (RN) license, OR
  • Degree in Social Work from an accredited institution.
  • Experience working with individuals with intellectual/developmental disabilities, behavioral health needs, or complex medical needs preferred.
  • Experience in residential services, healthcare, behavioral health, case management, or a related human-services environment preferred.
  • Strong knowledge of person-centered planning and behavioral support principles.
  • Strong organizational, communication, documentation, and problem-solving skills.
  • Ability to manage multiple priorities and respond appropriately to urgent participant and staffing concerns.
  • Ability to conduct investigations, identify deficiencies, analyze trends, and implement corrective actions.
  • Ability to train, coach, and provide constructive feedback to managers and direct-care staff.
  • Working knowledge of electronic documentation, scheduling, medication-management, and quality-assurance systems preferred.
  • Ability to maintain confidentiality and comply with applicable privacy requirements.
  • Must successfully complete all required background checks, clearances, and agency-required training.

Physical and Work Requirements

The Clinical Manager must be able to perform the essential functions of the position, with or without reasonable accommodation, including traveling between residential locations as needed, attending participant meetings and appointments, reviewing electronic and written records, conducting audits, participating in training, and responding to operational and clinical concerns.
