What You’ll Do
As a Home Health RN, you will make a direct difference in patients' lives by delivering exceptional care where they are most comfortable—at home.
- Lead Patient Care: Work directly with physicians to manage medication needs, adjust care plans, and help prevent re-hospitalizations.
- Coordinate the Team: Manage and guide LPNs, aides, and therapy staff to deliver holistic care tailored to each patient.
- Empower Families: Educate and counsel family members on care processes, disease management, and recovery.
- Seamless Documentation: Complete OASIS, recertifications, discharges, and ROCs within our EMR system.
- Grow & Connect: Participate in team orientation, staff meetings, and ongoing CEU opportunities.

Requirements:What We’re Looking For
- Active Michigan RN License (Required)
- Current CPR Certification & Valid Auto Insurance (Required)
- Previous home health or related nursing experience preferred.
- Strong communication, organization, and EMR skills.
