Job Summary

Affinity Care MN is hiring a mission-driven Waiver Case Manager. We support people throughout the community, whether they live in their own homes or in our residential settings. Our mission is to improve quality of life by empowering people with knowledge, resources, and skills, and by treating everyone with dignity and respect.

The Waiver Case Manager delivers person-centered case management to individuals living with serious and persistent mental illness, substance use disorders, and co-occurring medical conditions. These individuals qualify for services through county-administered waiver programs, including Community Access for Disability Inclusion (CADI), Developmental Disabilities (DD), Alternative Care (AC), Elderly Waiver (EW), and Brain Injury (BI).

Job Responsibilities

  • Carry and maintain a caseload of individuals diagnosed with serious and persistent mental illness, substance use disorders, and co-occurring medical conditions across the assigned county.
  • Meet with clients at least monthly, and twice per year as required by waiver-specific rules.
  • Manage client intake and discharge within established parameters and timelines.
  • Work alongside other professionals, including physicians, mental health providers, and care teams, to build and update each client's plan.
  • Confirm that financial authorizations and provider contracts are in place.
  • Document services in each individual's record so information stays accessible, outcomes can be measured, and record-keeping standards are met.
  • Connect individuals with community and housing resources that fit their needs and preferences.
  • Deliver crisis assessment and intervention so individuals stay safe in their home and community.
Requirements:

Must have one of the following:

The Waiver Case Manager must meet Minnesota's qualification standards, which require one of the following:

  • A bachelor's degree from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field; OR
  • A bachelor's degree from an accredited four-year college in any field, plus one year of experience as a social worker, case manager, or care coordinator at a public or private social service agency.
  • Willingness to travel throughout the Metro Area (mileage reimbursement applies), with a current driver's license, clean driving record, and a personal, insured vehicle suitable for transporting clients.

Must also meet the following:

  • Maintain an acceptable motor vehicle record.
  • Must not be disqualified by the required DHS criminal background study.
  • Complete all orientation and training as written in policy.
  • Good oral and written communication and organizational skills.
  • Proficiency with office productivity software platforms.

Preferred Qualifications

  • One to two years of previous waiver case management experience.
  • Experience delivering direct services to adults with serious and persistent mental illness.
  • One year of experience providing case management services to persons with developmental disabilities.

Working Conditions

This position travels regularly into the community and works in a variety of settings, both on and off site. The position is subject to frequent interruptions, crisis management, and imposed deadlines. The Case Manager has regular contact with clients, family members, friends, federal, state, and county regulatory personnel, other provider personnel, and the general public, some of whom may be hostile or emotionally upset. The position may be exposed to verbally aggressive individuals, infectious diseases, odor, and dust.

Physical & Mental Requirements:

Employees in this role must be able to:

  • Walk, stand, sit, bend, climb stairs, and perform crisis prevention/intervention techniques
  • Use hands for writing, typing, and data entry
  • Lift up to 40 lbs. and occasionally exert physical effort during duties
  • Maintain vision and hearing necessary for reading charts, operating office equipment, using a computer, and driving a vehicle
