Job Description
Attendant Care (ATC) Provider – 32 Hours/Week
Position: Attendant Care (ATC) Provider
Hours: 32 hours per week
Schedule:
Saturday: 7:00 AM – 3:00 PM
Sunday: 7:00 AM – 3:00 PM
Tuesday: 3:00 PM – 11:00 PM
Thursday: 3:00 PM – 11:00 PM
Divine Group Management LLC is seeking a dependable and compassionate Attendant Care Provider to provide direct support to an individual receiving in-home ATC services.
Responsibilities
1. Provide direct supervision and support according to the member’s service plan.
2. Assist with daily living and personal care needs as required.
3. Maintain the required level of supervision throughout the scheduled service hours.
4. Promote the member’s safety, independence, dignity, and well-being.
5. Complete required documentation and EVV accurately and on time.
6. Follow all DDD, agency, and member-specific policies and procedures.
7. Communicate appropriately with the member, guardian, and agency management.
Requirements:
* Must meet all Arizona DDD Direct Care Worker requirements.
* Current CPR/First Aid certification.
* Article 9 certification or ability to complete required training.
* Valid Level One Fingerprint Clearance Card.
* Reliable transportation and dependable attendance.
* Ability to consistently work the posted schedule.
