Job Description
Job Summary:
The Patient Care Coordinator is accountable and responsible for managing the clinical care of course of treatment of a number of patients. Coordination of care is accomplished through collaboration with physicians, associate staff, and other health care providers to ensure that patient outcomes are achieved within the projected course of treatment using the nursing process. The Patient Care Coordinator is accountable to the Nurse Manager. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed.
Performance Expectations:
· Demonstrates the ability to perform the expectations of the ED RN.
· Demonstrates the ability to generate a daily staffing schedule according to approved matrix, and communicate with manager when unable to do so.
· Demonstrates the ability to coordinate safe patient care.
· Demonstrates the ability to convey issues of a complex nature in a clear concise and organized manner.
· Demonstrates the ability to prioritize and delegate task effectively and efficiently to the staff per facility and Mississippi State Board of Nursing practice.
· Demonstrates ability to evaluate staff competencies and provide training and/or education as needed to ensure safe patient care.
· Demonstrates the ability to formulate and implement staff assignments to ensure safe patient care.
· Demonstrates the ability to supervise staff performance, providing feedback as appropriate.
· Demonstrates the ability to prioritize task and manage time appropriately.
· Demonstrates the ability to serve as a role model.
· Demonstrates the ability to recognize task appropriate to delegate and when.
· Demonstrates the ability to demonstrate skillful conflict resolutions.
· Demonstrates the ability to promote and support team work.
· Demonstrates the ability to review the EMR for abnormal results and communicate with the ERP in further treatment is needed.
· Demonstrates the ability to effectively and efficiently flow the department and coordinate with all appropriate disciplines to do so.
Qualifications:
Education/Skills
Graduate of a recognized non-online RN program required. A minimum of 8 hours education regarding current stroke management required during orientation and 8 hours annually that (to include 3 hours of NIHSS certification
Work Experience:
Two years as a staff RN in an acute setting preferred.
Certification/Licensure-DUE UPON HIRE
· Licensed RN able to practice within the State of MS Additional Certification/Licensure - Obtained based on required timeframe below
· Basic Life Support Within 30 Days of Employment Required
· Advanced Cardiac Life Support Within 90 Days of Employment Required
· Moderate Sedation Within 90 Days of Employment Preferred
· Pediatric Advance Life Support Within 180 Days of Employment Required
· Trauma Nursing Core Course Within 1 Year of Employment Required
· Telemetry Nurse Education Within 90 Days of Employment Required
· NIH Stroke Scale Within 90 Days of Employment Required
· Neonatal Resuscitation Within 90 Days of Employment Preferred
