Blue Stone Therapy is seeking a Physical Therapist Assistant (PTA) to serve as Rehab Director at Mapleton Community Home in Mapleton, MN.
This is a hands-on leadership opportunity for a PTA who wants to continue providing patient care while taking the next step in their career. You'll lead the day-to-day operations of the rehabilitation department, support and mentor the therapy team, and build strong relationships with facility leadership - all while remaining actively involved in patient care.

What You’ll Do
As Rehab Director, you'll combine direct patient care with day-to-day leadership of the rehabilitation department. You'll help keep the team organized, supported, and focused on delivering high-quality patient care.
Responsibilities include:
- Leading daily operations of the rehabilitation department
- Coordinating staffing, scheduling, caseloads, and department workflow
- Providing direct patient care as a Physical Therapist Assistant
- Coaching, mentoring, and supporting therapy team members
- Collaborating with facility leadership and interdisciplinary team members
- Supporting quality patient outcomes and a positive patient experience
- Monitoring documentation, regulatory, and company requirements
- Helping identify opportunities to strengthen therapy services and department performance
- Graduate of an accredited Physical Therapist Assistant program
- Current Minnesota Physical Therapist Assistant license or eligibility for licensure
- Minimum of one year of experience as a Physical Therapist Assistant
