Job Description

Community Partnerships Manager

This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Community Partnerships Manager based in United States.

This role is focused on building and scaling high-impact partnerships across the healthcare ecosystem, connecting clinical innovation with the organizations that shape population health outcomes.
You will serve as a trusted external representative, developing relationships with health systems, ACOs, CINs, and community-based organizations to expand access to care and strengthen referral pathways.
The position blends strategic business development with relationship management, requiring strong executive presence and the ability to influence senior healthcare leaders.
You will translate value-based care objectives into actionable partnership models that improve patient outcomes and system performance.
Working in a highly mission-driven and fast-evolving healthcare environment, you will help embed services into regional healthcare networks.
This is a high-ownership role for someone who thrives in complex stakeholder environments and is motivated by measurable social impact.

Accountabilities:
  • Develop and execute regional partnership strategies across healthcare systems, ACOs, CINs, and community organizations.
  • Identify and engage senior healthcare stakeholders, including executives responsible for population health, care management, and clinical operations.
  • Convert strategic relationships into structured referral pipelines and formalized partnership workflows.
  • Manage a portfolio of institutional partners, ensuring strong engagement, operational alignment, and ongoing satisfaction.
  • Track partnership performance, referral activity, and operational metrics, and deliver regular business reviews to stakeholders.
  • Represent the organization at healthcare conferences, policy forums, and industry events to strengthen market presence.
  • Monitor regional healthcare trends, including value-based care models and Medicaid-related ecosystem developments.
  • Collaborate cross-functionally to resolve operational challenges and improve partnership effectiveness.

Requirements:

  • 3–5+ years of experience in B2B account management, healthcare partnerships, or community-based organization engagement.
  • Proven ability to influence and build partnerships without financial leverage, particularly in complex healthcare environments.
  • Strong understanding of value-based care, population health, and how ACOs, CINs, and managed care organizations operate.
  • Experience managing multiple stakeholders across matrixed environments with competing priorities.
  • Excellent communication, negotiation, and presentation skills with executive-level stakeholders.
  • Ability to translate healthcare strategy into actionable partnership execution and measurable outcomes.
  • Familiarity with healthcare CRM tools (e.g., Salesforce) and EHR referral workflows is a plus.
  • Prior exposure to healthcare systems, health plans, or community health organizations strongly preferred.
  • Willingness to travel as required, particularly within regional markets.

Benefits:

  • Competitive base salary range (approximately $115,000–$130,000 annually)
  • Monthly travel/car stipend to support field and partner engagement activities
  • Comprehensive medical, dental, and vision insurance coverage
  • Mental health support services and employee assistance programs
  • 4 weeks of annual paid vacation, increasing with tenure
  • Paid sick leave and 9 company holidays per year
  • 12 weeks of fully paid parental leave after eligibility period
  • 401(k) retirement savings plan
  • Remote-first work model with equipment provided
  • Additional wellbeing, fertility, and family support programs
