CLINICAL PHARMACIST AMBULATORY - ORRVILLE

Position Summary

The primary purpose of the Ambulatory Clinical Pharmacist is to work closely with patients and their healthcare providers to address comprehensive health needs through a collaborative, patient-centered approach. The clinical pharmacist optimizes medication regimens through initiation, adjustment, and/or de-prescribing of medications.

The Aultman Health System MEDS Clinics are ambulatory clinics that focus on improving patient care through education and chronic disease state management.

Qualifications

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy degree (PharmD) from a college or a university accredited by the American Association of Colleges of Pharmacy.
  • Pharmacy License (RPh) in the State of Ohio, Board Certification or eligibility within 1 year.
  • Completion of ASHP accredited PGY-1 Residency. Minimum one-year related experience preferred.
  • Obtain a personal National Provider Identifier (NPI) within 30 days of hire.
  • Obtain and maintain current American Heart Association (AHA) training in Basic Life Support (BLS) utilizing Resuscitation Quality Improvement (RQI) is required.

Responsibilities & Expectations

  • Maintains proficiency in clinical pharmacy programs and pharmacy operations as applicable to clinical specialty/role.
  • Places priority on the delivery of patient-centered care and establishes collaborative pharmacist-patient relationships.
  • Provide comprehensive medication management for a wide variety of patients with chronic health conditions.
  • Initiate, adjust and/or discontinue medications, order and interpret labs and perform limited physical assessments under a collaborative practice agreement.
  • Perform point-of-care testing and adjust therapies to reach therapeutic goals.
  • Provide education on disease states, medications, medical devices, and lifestyle.
  • Participate in the development, implementation, and evaluation of patient care services.
  • Engage in wellness and health promotions, public health initiatives, and patient advocacy.
  • Ensure patients have access to affordable medications. Responsible for completing Prior Authorizations, managing Prescription Assistance Program applications, and utilizing available resources.
  • Tracking and interpreting patient data and quality metrics for clinic use.
  • Maintain proficiency in hospital computer systems and medication ordering systems, including documenting and appropriately billing patient encounters.
  • Maintain proficiency in pharmacy resident and student preceptor roles and actively emulate education/mentorship skills.
  • Develop student, resident and staff training experiences/competences and create new relationships for teaching and training opportunities.
  • Understand regulatory requirements and standards affecting his/her scope of practice.
  • Follow established standard word parameters and maintain standard work environments.
  • Perform other duties as assigned by pharmacy leadership.
