We are seeking a compassionate, skilled nurse who is passionate about delivering exceptional, person-centered care that enhances the safety, comfort, and overall well-being of every resident. In this role, you will thoughtfully assess resident needs, administer treatments and medications, and support individualized care plans that promote health, dignity, and quality of life. You will collaborate closely with CNAs, physicians, and families, while providing leadership and guidance across shifts to ensure seamless, coordinated care. With a strong commitment to safety, infection control, accurate documentation, and regulatory excellence, you will help foster a reliable, supportive, and well-organized care environment. The ideal candidate is adaptable, team-oriented, and deeply committed to making a meaningful difference in the lives of residents each day.

**Final compensation within the listed pay range will be based on relevant experience, skills, and qualifications, and will align with our established pay scale.**

Responsibilities:

  • Models a service attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workers
  • Ensure resident/patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazards
  • Maintain good attendance record
  • Demonstrates appropriate physical assessment skills for initial and follow-up appraisal of resident status. Possesses an adequate knowledge of clinical diseases
  • Demonstrates ability to perform nursing treatments and procedures as prescribed for residents. Administers those treatments in an appropriate manner.
  • Demonstrates adequate knowledge of specialized diets and medications commonly prescribed to residents and administers appropriate medications
  • Demonstrates knowledge and awareness of rehabilitative and preventative aspects of care and ensures resident safety, comfort and protection
  • Ensures that all residents under his/her direct supervision is provided care and services to attain or maintain the highest practical physical, mental and psychosocial wellbeing, i.e. maintain current weight, prevent pressure ulcers, maintain or improve self-care abilities.
  • Supervises and directs care givers to residents by Med. Aides and CNA's in an appropriate manner. Ensures continuity of care by appropriate directing, teaching and counseling the support staff
  • Practices standard infection control techniques
  • Promptly reviews, revises and confirms staff assignments at the beginning of the shift, to ensure all residents are assigned a caregiver. Make rounds every 3-4 hours to assess the resident's clinical condition and productivity of the assigned staff
  • Notify MD, family, administrative staff and direct care providers of resident's change in condition and necessary follow-up
  • Ensures residents receive prescribed diets, monitors meal percentages, offers substitutes when appropriate, reports weight loss to Clinical Coordinators and MD, and provides appropriate interventions for weight loss
  • Complete admission, transfer and discharge information as indicated in Electronic Health Record
  • Ensures all emergency equipment is available and functional on assigned unit
  • Complete an incident report as indicated and ensure adequate follow up is completed until resident is stable and/or incident resolved
  • Documents notes on each resident reflecting skilled care, resident instruction and resident/family response, doctor notification, a specific plan for ongoing care in Electronic Health Record
  • Submits written plans of treatment reflecting status and nursing care plans in a timely and accurate manner
  • Follows documentation procedures as required by Forefront Living, State and Federal regulations. Uses accurate terminology
  • Completes census information as required
  • Communicates unresolved work-related problems using chain of command, consistently demonstrates effective problem-solving skills and handles confidential information appropriately
  • Demonstrates flexibility with a changing workload/caseload, sets priorities accordingly and carries out job assignments in a self-directed manner
  • Other duties as assigned

Qualifications:

  • High school diploma or G.E.D.
  • Valid Texas LVN license
  • Ability to perform Essential Duties
  • Genuine caring for and interest in the care of mature adults, persons with disabilities and hospice patients
  • Ability to perform work tasks within the Physical Demand Requirements
  • Ability to comply with the Resident Bill of Rights and Employee Responsibilities
  • Ability to comply with organizational and departmental safety policies and procedures
  • Demonstrated supervisory and managerial skills
  • Knowledge of current professional nursing theory and practice
  • Excellent technical, assessment and documented skills
  • Ability to organize workflow, respond to emergencies and note detail
  • Ability to maintain a positive, stable disposition in performing work in association with staff and residents.
  • Ability to maintain a high level of performance in an environment of frequent demands and changes
  • Excellent verbal and written communication skills
  • Ability to read, write and effectively communicate in English
  • Excellent computer skills
