Schedule: Full-Time or Part-Time Available + On-Call Rotation

Requirements:

Responsibilities

Conduct comprehensive nursing assessments and ongoing patient evaluations.

Develop and implement individualized hospice care plans.

Provide pain and symptom management.

Perform wound care and skilled nursing interventions.

Educate patients, caregivers, and families regarding disease progression and hospice services.

Coordinate care with physicians and interdisciplinary team members.

Maintain accurate and timely documentation.

Participate in on-call rotation as assigned.

Advocate for patient comfort, dignity, and quality of life.

Qualifications

Current Minnesota Registered Nurse (RN) License.

Hospice experience preferred.

Experience in ICU, oncology, palliative care, geriatrics, home health, or end-of-life care strongly preferred.

Strong assessment and critical thinking skills.

Excellent communication and interpersonal skills.

Ability to work independently while collaborating effectively with the care team.

Valid driver's license and reliable transportation.
