LVN Lead Care Manager in Irvine, California at Sowing Seeds Health Inc

The LVN Lead Care Manager will play a vital role in coordinating patient care across healthcare settings. This position combines clinical nursing expertise with case management principles to ensure patients receive appropriate, timely, and cost-effective care. The LVN Case Manager collaborates with physicians, social workers, and other healthcare professionals to develop and implement individualized care plans.

Key Responsibilities
Conduct comprehensive assessments of patients' medical, psychosocial, and functional needs
Develop and implement individualized care plans in collaboration with interdisciplinary teams
Monitor patient progress and adjust care plans as needed
Facilitate transitions of care, including discharge planning and referrals to community resources

Perform utilization reviews to ensure appropriate levels of care
Educate patients and families on treatment plans, medications, and follow-up care
Maintain accurate and timely documentation in electronic health records
Advocate for patients to ensure access to necessary services and support
Participate in quality improvement initiatives and case review meetings

Qualifications
Active LVN license in California
Minimum of 2 years of clinical nursing experience, with at least 1 year in case management preferred
Strong understanding of healthcare systems, insurance plans, and community resources
Excellent communication, organizational, and critical thinking skills
Proficiency in electronic medical records (EMR) and case management software

Full-Time 9AM -1PM (lunch 1 to 2) 2PM-6PM
Vacation: 40 hours per year and 40-hour sick time; accumulated at 1 hour per 30 hours worked. Ten paid holidays. (Thanksgiving Day and the Day after, Christmas Eve and Christmas Day, New Year’s Day, Martin Luther King’s birthday, Juneteenth, 4 of July, Labor Day, and Memorial Day)
