Registered Nurse (RN) – Weekend Overnight Case in Philadelphia, Pennsylvania at Affinity HealthCare Solutions Skilled Care LLC

Affinity Home Care is seeking a compassionate, dependable, and skilled Registered Nurse (RN) to provide one-on-one care for a patient located in Philadelphia, PA 19138.

We are currently staffing a weekend overnight case with the following scheduling options:

11:00 PM – 3:00 PM (16-hour shift)
11:00 PM – 7:00 AM
7:00 AM – 3:00 PM
Candidates may work Saturdays, Sundays, or both based on availability.

The ideal candidate will have strong clinical skills, excellent documentation practices, and a commitment to delivering high-quality patient care in the home setting.

Responsibilities
- Perform comprehensive patient assessments
- Monitor and document patient condition and vital signs
- Administer medications and treatments as ordered
- Collaborate with physicians and the care team
- Provide patient and family education
- Maintain accurate documentation in compliance with agency standards
- Respond appropriately to changes in patient condition
- Ensure a safe and comfortable environment for the patient

Qualifications
- Active Pennsylvania RN License
- Current CPR Certification
- Home Health or Private Duty experience preferred
- Strong clinical assessment and communication skills
- Ability to work independently and manage patient care effectively
- Reliable transportation
