Schedule:PRN Position Summary: Bridging Life Hospice is seeking a compassionate and dedicated Hospice Registered Nurse (RN) to provide high-quality, patient-centered care to individuals facing end-of-life for our inpatient unit team. This role offers autonomy, meaningful connections, and the opportunity to make a profound impact on patients and their families during one of life’s most critical transitions. Key Responsibilities: Collect and document comprehensive health status data using patient records, family input, and available resources. Formulate, implement, and evaluate individualized plans of care in accordance with agency policies. Prioritize and coordinate care in a responsive and autonomous manner. Provide emotional and clinical support to patients and families during the terminal phase of illness. Perform rounds with physicians and communicate any care changes to the hospice team. Collaborate with and supervise Hospice Aides/HHAs, adjusting care plans as needed and providing shift-by-shift guidance. Participate in interdisciplinary team (IDT) meetings and submit timely documentation. Support performance improvement efforts by responding promptly to documentation and information requests. Serve as a resource and positive role model for team members and new staff. Actively participate in cost-effective care strategies, time management, and safe work practices. Communicate clearly and efficiently with physicians, caregivers, team members, and community partners. Maintain knowledge of hospice nursing best practices and pursue continued professional development. Represent Carroll Hospice and Carroll Hospital in the community through liaison activities. Required Qualifications: Active Registered Nurse (RN) license through the Maryland Board of Nursing (MBON) AHA BLS Provider certification Reliable transportation Why Join Us?
