About the Role
Burke is seeking a Service Coordinator to provide ongoing support to individuals enrolled in the HCS, TxHmL, and General Revenue Programs. This role is essential in ensuring individuals receive appropriate services, support, and resources to improve their quality of life.
The Service Coordinator works closely with individuals, families or legally authorized representatives, and community partners to develop and monitor service plans.
What You'll Do
- Conduct assessments and complete required documentation
- Facilitate service planning and program development
- Monitor services and ensure quality and compliance
- Link and refer individuals to appropriate community resources
- Provide crisis prevention and intervention support
- Collaborate with families, team members, and external providers
- Travel within the service area to meet with individuals in homes, schools, or day service settings
- Maintain accurate and timely documentation
What We're Looking For
Required:
- Bachelor's degree in Social Work, Psychology, Counseling, Sociology, Human Development, Criminal Justice, or related Human Services field
- Valid Texas driver's license
- Reliable transportation and ability to travel within the service area
- Insurable driving record
- Ability to complete required training (SAMA, CPR, First Aid, Defensive Driving)
Preferred:
- Strong typing skills and proficiency in Microsoft Word
- Excellent organizational skills and ability to meet strict deadlines

Work Schedule
- Monday-Friday, generally 8:00 AM - 5:00 PM

Compensation & Benefits
- Salary: $42,120 - $43,555 annually (based on qualifications and experience)
Physical Requirements
- Ability to lift up to 25 lbs.
- Bending, reaching, and mobility throughout the workday
- Visual and auditory ability to assess consumer needs
- Manual dexterity for writing and typing
- Reliable transportation and driving required
