The Designated Coordinator (DC) is responsible for overseeing and coordinating services for individuals receiving support under the agency’s 245D program. This role ensures service delivery is person-centered, compliant, organized, and responsive to client needs. The DC plays a key leadership role in coordinating care, supporting staff, maintaining documentation, and promoting quality outcomes for the individuals served.

Core Responsibilities

The Designated Coordinator is responsible for:

  • Coordinating service delivery for individuals with developmental disabilities and related support needs in accordance with all regulatory requirements, service agreements, support plans, and agency policies
  • Developing, implementing, and monitoring individualized service plans and provider-developed plans that promote independence, community integration, safety, and personal growth
  • Participating in intake, assessments, service planning meetings, 45-day meetings, 60-day meetings, and ongoing plan review processes
  • Serving as a primary contact for clients, families, guardians, case managers, community partners, and internal departments
  • Supervising and supporting direct support staff through training, mentorship, coaching, and performance follow-up to maintain quality care standards
  • Ensuring staff understand and follow client-specific plans, instructions, restrictions, goals, and delegated supports
  • Overseeing documentation processes including service plans, progress notes, incident reporting, compliance reports, and related records with accuracy and timeliness
  • Monitoring service quality, conducting file audits, identifying gaps, and implementing corrective actions or improvements as needed
  • Supporting quality assurance efforts through regular review of documentation, service delivery practices, and compliance standards
  • Managing assigned caseload responsibilities efficiently and ensuring timely follow-up on signatures, assessments, authorizations, renewals, and plan updates
  • Building and maintaining strong working relationships with clients, families, staff, interdisciplinary team members, and community partners
  • Assisting with administrative duties necessary to support compliant and sustainable service delivery
  • Completing additional work outside regular business hours, including from home when necessary, to meet documentation deadlines, client needs, urgent follow-up, and compliance expectations

Authority & Scope

The Designated Coordinator has the authority to:

  • Coordinate day-to-day service delivery for assigned clients
  • Guide staff on client-specific supports, expectations, and implementation of plans
  • Communicate service needs, risks, concerns, and updates to the Designated Manager
  • Support staff training, competency development, and ongoing service coordination
  • Monitor files, documentation, and service delivery for compliance and quality
  • Escalate safety, medical, staffing, behavioral, or compliance concerns immediately
  • Assist with resource coordination and administrative follow-up needed to support continuity of care
  • Manage assigned caseloads and related administrative responsibilities in alignment with agency expectations

The DC does not independently override agency policy, authorize services outside approved parameters, or make decisions outside the scope of delegated authority.

The Designated Coordinator must meet applicable 245D qualification requirements and agency expectations, including:

  • Minimum of 3 years of experience as a Designated Coordinator preferred.
  • Proven management experience in social services, program development, human services, or related fields supporting individuals with developmental disabilities and/or behavioral health needs
  • Demonstrated ability to develop, implement, and evaluate programs that enhance client well-being while maintaining regulatory compliance
  • Strong relationship management skills to foster trust and collaboration among clients, families, staff, and community partners
  • Extensive administrative experience including documentation oversight, report writing, compliance follow-up, and coordination of service records
  • Experience supervising staff with a focus on team development, accountability, and performance support
  • Knowledge of behavioral health principles, person-centered practices, and best practices in supporting individuals with developmental disabilities
  • Excellent organizational, communication, and multitasking skills in a fast-paced environment
  • Ability and willingness to work additional hours, including from home when necessary, to fulfill the responsibilities of the role
