About This Position
Summary
The Manager of Post-Acute Strategy is responsible for leading the development, implementation, and oversight of strategic initiatives related to post-acute care across the health system. This leader will design, evaluate, and optimize partnerships, programs, and care pathways that improve patient outcomes, reduce readmissions, and ensure seamless transitions of care. The role requires a deep understanding of the post-acute continuum—including skilled nursing, inpatient rehabilitation, home health, hospice, and long-term care—along with strong skills in strategy, operations, and relationship management.
Responsibilities
Strategic Leadership and Planning: Develop and execute a comprehensive post-acute strategy aligned with the health system’s mission, goals, and population health objectives. Identify opportunities for growth, integration, and innovation within the post acute continuum of care. Establish key performance indicators (KPIs) to measure quality, efficiency, and patient outcomes.
Care Continuum Management: Strengthen care coordination and transition processes from acute to post-acute settings. Partner with hospital leadership, physicians, care management, and post-acute providers to reduce avoidable readmissions and improve patient experience. Evaluate and enhance clinical pathways, referral patterns, and network design.
Partnership Development: Build and manage strategic partnerships with skilled nursing facilities (SNFs), home health agencies, inpatient rehabilitation centers, hospice, and other post-acute providers. Provide insight with the value-based care team to negotiate and oversee preferred provider networks, joint ventures, and value-based arrangements. Maintain relationships with payers, community organizations, and regulatory bodies.
Operational and Financial Oversight: Support financial performance of post-acute programs, identifying cost[1]saving opportunities and optimizing utilization. Collaborate with finance, analytics, and contracting teams to develop value-based care models. Ensure compliance with federal, state, and accreditation requirements governing post-acute services.
Leadership and Collaboration: Provide directions and mentorship to teams involved in care transitions, population health, and post-acute programs. Serve as a subject matter expert and resource for executive leadership, clinical teams, and community partners. Drive change management efforts to embed best practices across the system
REQUIREMENTS:
3-5 years experience
Preferred: Master's Degree Master’s degree in Social Work, Public Health, Healthcare Administration, or Bachelor’s Degree in Nursing
Required Licenses And Certification: Curr MD RN, Social Worker, Counselor or Psychologist License
Preferred: Basic Life Support Care Management Certification within 3 years hire (CCM – Certified Case Manager)
