Licensed Practical Nurse (LPN) – Skilled Nursing Facility

About the Role

As a Licensed Practical Nurse, you will provide hands‑on nursing care under the direction of an RN Supervisor or the Director of Nursing. You’ll support residents with their daily healthcare needs, ensure compliance with regulatory standards, and help maintain the highest level of nursing practice within our facility.

Your role is essential in creating a safe, supportive, and compassionate environment for the residents who call our community home.

What You’ll Do
  • Deliver skilled nursing care to residents in accordance with care plans and regulatory guidelines

  • Assist in monitoring resident conditions and reporting changes to the RN Supervisor

  • Support residents with daily healthcare needs and medication administration

  • Maintain accurate documentation and uphold facility and state standards

  • Collaborate with the interdisciplinary team to ensure resident well‑being

  • Promote a positive, respectful, and team‑oriented work environment

What We’re Looking For

Education

  • Graduate of an accredited school of practical nursing

Experience

  • Minimum of one (1) year nursing experience

  • Skilled nursing facility experience preferred

Qualifications

  • Current, active Nursing License

  • CPR certification

  • Strong communication and organizational skills

  • Commitment to providing compassionate, resident‑centered care
