Job Description
$65,000 + depending on case load
We are looking for an eager and compassionate case manager to join our Behavioral Health Home Case Management team! As we grow and continue to build new partnerships in the community we are adding to staff and seek a case manager who is willing to assist and support children, adults and families in Knox and Waldo Counties.
The right candidate will truly love helping people and will have exceptional time management skills. They must have their MHRT C and a bachelors degree as well as a valid drivers license and reliable vehicle to see clients in person.
Pay is determined by case load and varies.
Responsibilities
· Coordinate and provide care that is safe, timely, effective, efficient, and client-centered
· Handle case assignments, write treatment plans and progress notes, review case progress and determine case closure
· Help clients achieve wellness and autonomy
· Facilitate multiple care aspects (case coordination, information sharing, etc.)
· Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
· Develop effective working relations with team members throughout the entire case management process
· Meet with your clients regularly in their home and local communities
· Link clients and families with local resources, agencies and providers

· Bachelor's in human services or related field
· MHRT-C required
· Minimum of 1 year of case management experience, must have worked with children and adults
