Job Description
Statement of Purpose:
To render professional nursing care to patients by diagnosing and treating human responses to actual or potential health problems. Performs a variety of direct and indirect patient care duties as qualified by licensure, orientation and competency.
MAJOR TASKS, DUTIES AND RESPONSIBILITIES
- Maintains the standards of nursing care in the practice and implements the policies and procedures.
- Assesses the patient’s condition and nursing needs; sets goals; prescribes appropriate nursing actions to meet those goals and physical, psychological, social and rehabilitative needs of the patient.
- Reports pertinent observations and reactions regarding patients to the appropriate person (i.e. physician/physician extender, or clinic coordinator).
- Administers medications at the direction of the physician. Transmits prescriptions to providers for pharmacy processing and documents the information as required in the patient’s medical record.
- Assists provider with special tests and procedures as mandated by the specialty of the practice.
- Participates in economical utilization of supplies and ensures that equipment is maintained in a clean and safe manner.
- Orders and stocks supplies as needed.
- Participates in orientation of new personnel.
- Assists team members in giving care to patients or administers direct care when professional nursing skills and judgment are needed.
- Establishes and maintains communications and utilizes teaching opportunities with patients and other significant individuals
- Assists with or institutes emergency measures for sudden adverse developments in patients such as cardiac arrest.
- Obtains prior authorizations, makes referral appointments for specialists as needed.
- Demonstrates knowledge and skills necessary to provide care appropriate to all ages of patients as documented by training and competency assessment.
- Promotes health and safety concepts to prevent illness or injury to self, co-workers or patients. Takes preventive measures as appropriate.
- Collects specimens for lab analysis and performs waived testing per JMH protocol.
- Participates in quality improvement initiatives as part of the practice care team.
- Ensures job functions align with office procedures, policies and patient-centered medical home standards.
- When staffing needs dictate, may work hours outside of normal schedule and/or in another practice.
- Performs duties and assists providers as part of the practice care team. Takes part in daily team huddles and/or periodic team meetings to facilitate communication and enhance patient experiences and quality delivery of care.
- Delegate specific tasks to other qualified personnel, such as nurses or medical assistants, by providing clear instructions, supervision and evaluation of completed tasks.
EDUCATION: Graduate of an accredited school of nursing. New York State licensure and current registration as Registered Nurse required. Current CPR/BLS certification required.
EXPERIENCE: One-year acute care and/or one year doctor office experience.
CONTACTS: Frequent contact with patients, family members, physicians, and other employees.
PHYSICAL DEMANDS: Stands and walks most of the day.
WORKING CONDITIONS: Normal. Frequent exposure to disagreeable odors, cuts, burns, infections, communicable diseases, strains and injuries from patients and equipment.
