Position Summary

The Clinical Assessment Registered Nurse (RN) performs in-home admissions, Start of Care (SOC), reassessments, supervisory visits, and other clinical evaluations. This role requires excellent clinical judgment, strong documentation skills, and a commitment to delivering compassionate, high-quality home health services while ensuring compliance with all applicable regulations.

What You Will Do
  • Complete comprehensive in-home nursing assessments and admissions.
  • Perform SOC, ROC, supervisory, reassessment, and discharge visits.
  • Develop and update individualized plans of care.
  • Identify changes in patient condition and communicate with physicians promptly.
  • Educate patients and caregivers on diagnoses, medications, and safety.
  • Coordinate care with physicians, therapists, pharmacies, and office staff.
  • Complete accurate documentation within agency timeframes.
  • Ensure compliance with VDH, DMAS, Medicare/Medicaid (as applicable), HIPAA, and agency policies.
  • Participate in orientation, meetings, and continuing education.
What We Are Looking For
  • Active, unrestricted Virginia RN license.
  • Current CPR/BLS certification.
  • Minimum one year of nursing experience (home health preferred).
  • Excellent assessment, communication, and critical thinking skills.
  • Ability to work independently and manage a flexible schedule.
  • Reliable transportation, valid driver's license, and current auto insurance.
  • Ability to travel throughout the assigned service area.
  • Proficiency with electronic documentation (Alora experience is a plus).
  • Successful completion of background screening and any pre-employment requirements.

Employment Information

Employment Type: PRN, Part-Time, or Full-Time
Schedule: Monday–Friday with occasional evenings, weekends, or on-call assignments as needed.
