Concept Rehab is seeking a dedicated Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team in a skilled nursing and post-acute care setting. In this role, you'll work closely with Occupational Therapists and interdisciplinary care teams to help patients improve function, enhance safety, and achieve greater independence in their daily lives.

Requirements:

What You'll Do

  • Implement individualized treatment plans designed to improve functional performance
  • Provide skilled occupational therapy interventions that support independence with daily activities
  • Assist patients with self-care, dressing, grooming, bathing, and other activities of daily living (ADLs)
  • Support upper extremity strengthening, fine motor coordination, balance, and functional mobility activities
  • Monitor patient progress and communicate changes to supervising therapists
  • Collaborate with nursing staff, rehabilitation team members, patients, and families
  • Educate caregivers and facility staff on therapeutic techniques and safety recommendations
  • Contribute to discharge planning and continuity of care efforts
  • Maintain timely, accurate, and compliant electronic documentation
  • Promote a positive and engaging rehabilitation experience for patients

Qualifications

  • Graduate of an accredited Occupational Therapy Assistant program
  • Active Ohio COTA license in good standing
  • Skilled nursing, geriatric, or post-acute care experience preferred
  • Strong interpersonal and communication skills
  • Ability to work effectively within a multidisciplinary healthcare team
  • Compassionate, patient-focused approach to care
