Easterseals is seeking a dedicated and compassionate Case Manager – Autism Services to support individuals with Autism Spectrum Disorder (ASD) and their families. This role serves as a key advocate, liaison, and educator, helping clients navigate the continuum of care from screening and assessment through service coordination and discharge.
The Case Manager plays a critical role in ensuring families have access to the resources, education, and support needed to achieve positive outcomes.

What You'll Do:
• Welcome families, build rapport, and serve as their primary point of contact throughout the assessment and service process
• Complete autism screenings and providing education and support based on results
• Act as client advocate regarding intervention and treatment options
• Manage a client caseload, identify goals and develop individualized service plans
• Coordinate referrals to Easterseals programs and community resources
• Collaborate closely with physicians, therapists, internal teams, and community partners to ensure quality, timely, person-centered care
• Conduct and participate in community outreach and educational initiatives, including workshops, events, and awareness efforts.
• Ability to participate in community outreach events, which may require standing or walking for extended periods of time.
• Ability to lift, carry, push, and/or pull up to 50 pounds and perform physical event setup tasks, including reaching, overhead and handling equipment (e.g., tents, tables, and outreach supplies).
• Maintain accurate documentation in the electronic medical record and assist with assigned data tracking and reporting
• Stay informed on current research, services, and community resources to provide up-to-date information
What You Bring:
• Bachelor’s degree in Social Work, Psychology, or a related human services field (required)
• At least 1 year of social work or human services experience
• Knowledge of Autism Spectrum Disorder (required)
• Knowledge of community resources and family supports (preferred)
• Ability to collaborate with multidisciplinary teams and communicate effectively with families
• Strong organizational skills, initiative, and flexibility
• Ability to effectively communicate information both verbally, in writing, and through real-time handwritten documentation when needed.
• Proficiency in EMR systems and Microsoft Office
• Professionalism, integrity, tact, and good judgment
• Ability to meet our approved driver qualifications in order to drive your personal vehicle, as required, for program-related needs (minimum age of 21 and good driving history)
•Ability to lift up to 30 pounds
•Ability to flex hours to include occasional evenings and Saturdays
