Position Title: Hospice Aide
Department: Clinical/Patient Care
Reports To: Registered Nurse
Position Summary
A Hospice Aide provides physical and emotional support to patients and their families, assists with daily living activities, and ensures patient comfort and dignity.

Essential Duties and Responsibilities? Assist patients with activities of daily living, such as bathing, dressing, and eating
? Offer companionship, emotional support, and comfort to patients and their families
? Observe and report any changes in the patient's physical or mental condition to the RN Case Manager/Administrator
? Ensure a clean, safe, and comfortable environment for the patient and family. Report safety concerns to RN Case Manager/Administrator
? Help with light housekeeping duties, such as laundry and meal preparation
? Communicate effectively with patients, families, and other healthcare professionals
? Maintain accurate and detailed records of patient care
? Participate in required training and continuing education
Requirements:Qualifications
? Certified Nursing Assistant (CNA) or equivalent
? Experience in providing end-of-life care in a home care or other healthcare settings is preferred
? A current CPR certification is required
? Patience, compassion, and the ability to work effectively with patients and their families
? Ability to follow instructions and work independently in patient's homes
? Must have access to a reliable vehicle with a valid driver’s license in the state residing as well as proof of vehicle insurance
? Understanding and adherence to infection control protocols
? Must be able to show adaptability to patient's home environment
Work Environment
? Diverse surroundings in patient homes with unpredictable environments such as uneven surfaces and potential hazards such as animals, tobacco smoke and exposure to communicable diseases, etc.
? Hospitals, nursing homes, assisted living facilities, adult foster homes, etc.
Physical Demands
? Strong communication and interpersonal skills are essential
? Ability to assist patients with mobility and other physical tasks
? Ability to lift at least 50lbs and move patients as needed
? Ability to sit, stand, bend and walk for extended periods.
